Abstract

Introdução: Condrossarcomas são tumores ósseos malignos, caracterizados pela produção de matriz cartilaginosa. Vão desde tumores de baixo grau com pequeno potencial metastático até tumores de alto grau, agressivos e com potencial metastático elevado e precoce. Os tumores de baixo grau possuem poucas anormalidades no cariótipo e são quase diplóides, enquanto os tumores de alto grau resultam de cariótipos complexos e são aneuplóides. As aberrações cromossómicas mais frequentes provocam sinalização hedgehog constituitiva. Aqui, é apresentado o caso de um condrossarcoma do osso hióide com descrição imagiológica, cirúrgica e patológica. Acompanhado por uma revisão sumária da literatura. Apresentação do Caso: Homem de setenta e três anos apresentava uma massa cervical volumosa, localizada ao nível submentoniano. A TC mostrou uma massa bem circunscrita com 5x4x3cm localizada na região supra-hioideia. A peça operatória revelou um condrossarcoma de grau 2 originário do osso hióide. Discussão: O condrossarcoma do osso hióide é uma patologia rara, o que dificulta o seu diagnóstico. O desenvolvimento de marcadores moleculares pode desempenhar um papel importante no diagnóstico precoce e certeza do mesmo. A biópsia possui alta sensibilidade diagnóstica, mas o estadiamento correcto só é obtido em 46% das ocasiões. A TC é o método preferencial para caracterizar a extensão e origem tumoral. A excisão cirúrgica é o tratamento definitivo para o tratamento do condrossarcoma. A extensão da ressecção e a necessidade de terapia adjuvantes varia com o grau tumoral.

Introduction: Chondrosarcomas are malignant bone tumors that are characterized for producing cartilaginous matrix. They range from low-grade tumors with low metastatic potential to high-grade, aggressive tumors with early and high metastatic rate. Low-grade types have few karyotype abnormalities and are near-diploid, while high-grade chondrosarcomas originate from complex karyotypes and are aneuploid. The most common karyotype aberrations cause constitutive hedgehog signaling. Here, is presented a case of chondrosarcoma of the hyoid bone with imaging, surgical, and pathologic correlation, and a review of the literature. Case Presentation: Seventy-three years old man presented with a voluminous cervical mass located at submental level. The CT scan showed a well-circumscribed mass of 5×4×3cm located in the suprahyoid region. The operative finding revealed a grade 2 chondrosarcoma originating from the hyoid bone. Discussion: Chondrosarcoma of the hyoid bone is a rare pathology, thus making it a difficult diagnosis. The development of biomarkers would be an important step towards early detection and correct diagnosis of chondrosarcoma. Fine needle biopsies have high diagnosis accuracy, although correct grading is only obtained 46% of the time. CT scan is the gold standard to characterize tumor extension and origin. Surgical excision is the definitive treatment for chondrosarcoma. The extent of the surgery and need for adjuvant therapy varies with tumor grading.
