Estatinas e densidade mineral óssea

Abstract

Os inibidores da HMG-CoA redutase, também conhecidos como estatinas, são largamente usados para o tratamento da hiperlipidémia. Recentemente, estudos experimentais in vivo e in vitro revelaram que as estatinas têm um efeito protetor no tecido ósseo, através da expressão de BMP2 (bone morphogenetic protein-2) capaz de induzir a diferenciação osteoblástica. Vários estudos epidemiológicos também suportam esse efeito anabólico através da redução do risco de fractura e aumento da densidade mineral óssea particularmente em mulheres pós-menopausa. Neste estudo observacional transversal analisámos o efeito da terapêutica com estatinas na densidade mineral óssea em 74 indivíduos caucasianos do sexo masculino com idade >55 anos divididos em dois grupos: 59 doentes estavam sob estatinas numa duração superior a 3 meses e 15 doentes não realizavam terapêutica com estatinas. A densidade mineral óssea (DMO) foi medida ao nível da coluna lombar e fémur através de densitometria óssea e foi aplicado um protocolo clínico para avaliação de características demográficas e clínicas da população. Foi utilizada análise de covariância para avaliar a associação entre o uso de estatinas e a DMO, tendo em conta confundidores e co-variáveis relevantes. No presente estudo, o grupo dos doentes sob estatinas não mostrou alterações estatisticamente significativas na DMO quer ao nível da coluna lombar quer ao nível do fémur comparativamente ao grupo que não realizava estatinas, mesmo após ajustes. Os resultados obtidos não suportam um efeito major das estatinas no metabolismo mineral ósseo em homens de raça caucasiana.

The HMG-CoA reductase inhibitors, also known as statins are used widely for the treatment of hyperlipidemia. Experimental studies, both in vivo and in vitro, have suggested a protective effect of statins on bone tissue through the expression of BMP2 (bone morphogenetic protein-2) capable of inducing the osteoblastic differentiation. Several epidemiological studies support the anabolic effect of statins by reducing the risk of fracture and increasing bone mineral density particularly in post-menopausal women. In this cross-sectional observational study we examined the effect of statin therapy on bone mineral density of 74 Caucasian male subjects, aged> 55 years: 59 of them were under statins for more than 3 months, and 15 did not use statin therapy. Bone mineral density (BMD) was measured at the lumbar spine and femur using dual x-ray absorciometry, and a clinical protocol was applied to evaluate demographic and clinical characteristics of the population. Analysis of covariance was performed to evaluate the association between statins use and BMD, taking into account confounders and relevant covariates. In this study, the group of patients on statins for three months showed no significant differences in BMD both at the level of the lumbar spine, and of the femur comparatively to the group that did not perform statins, even after adjustments. Our results do not support a major effect of statins on bone metabolism in Caucasian men.
