Caso clínico de uma doente de 9 anos, do sexo feminino, diagnosticada com Histiocitose de Células de Langerhans sob a forma focal na mastóide direita no contexto clínico de uma dor persistente localizada à região mastoideia homolateral com sinais inflamatórios compatíveis com uma mastoidite subaguda, sem outra sintomatologia associada. A confirmação do diagnóstico foi feita pelo exame anátomo-patológico com análise histoquímica após estudo imagiológico indicativo de lesão lítica intraóssea a nível da porção mastoideia do osso temporal. A doente foi submetida a mastoidectomia simples direita mas com recorrência da sintomatologia pelo que iniciou terapêutica sistémica de acordo com as guidelines da Histiocyte Society (LCH-IV International Collaborative Treatment Protocol for Langerhans Cell Histiocytosis). Verificou-se uma boa resposta à terapêutica com vinblastina e prednisolona desde muito cedo com remissão da doença e sem sintomatologia até à data deste trabalho.

Clinical report of a 9 year old female patient diagnosed with Langerhans Cell Histiocytosis confined to the right mastoid in the context of a persistent pain localized to the right mastoid region and inflamatory signs compatible with a subacute mastoiditis, with no other symptoms. The anatomopathologic examination with the histochemical analysis confirmed the diagnose after the imagiological findings of lytic lesions inside the mastoid process of the temporal bone. The patient was submitted to a mastoidectomy but the disease relapsed, which justified a systemic therapy according to the Histiocyte Society guidelines (LCH-IV International Collaborative Treatment Protocol for Langerhans Cell Histiocytosis). Until the end of this report the patient’s disease went into remission with no symptoms or recurrences reported soon after the beginning of the treatment protocol with vinblastine and prednisolone.
