Repercussões cardiovasculares no adulto : pós reparação de tetralogia de Fallot

Abstract

Tetralogia de Fallot é a anomalia congénita cardíaca cianótica mais frequente. Com o avanço tecnológico e melhor vigilância, os indivíduos, após reparação desta cardiopatia, alcançam a idade adulta com boa qualidade de vida. No entanto, a longo prazo, apresentam condições anátomo-fisiopatológicas que lhes proporcionam alguma morbilidade. A regurgitação pulmonar é a mais frequente, causando dilatação e insuficiência ventricular direita. Ainda, é a causa de interações interventriculares anormais e alterações intrínsecas do miocárdio que originam focos arrítmicos ectópicos (circuitos de reentrada). Estes doentes culminam com apresentação de insuficiência cardíaca global ou morte súbita cardíaca por taquicardia ventricular. Ocorre, também, dilatação da raiz e aorta ascendente por aumento do stress hemodinâmico na parede arterial e por alterações histológicas que diminuem a sua rigidez. Esta anomalia estrutural também contribui para o desenvolvimento de insuficiência cardíaca pela presença de regurgitação aórtica. Para avaliação morfológica e funcional das estruturas cardiovasculares existem vários exames complementares de diagnóstico, dos quais se destaca a ressonância magnética cardíaca. Quanto à profilaxia e terapêutica, a implantação de cardioversor desfibrilhador para prevenção de morte súbita deve ser precoce e o tratamento da insuficiência cardíaca deve ser o mais ajustado. A substituição da válvula pulmonar precoce ao compromisso cardíaco irreversível é fulcral.

Tetralogy of Fallot is the most common form of cyanotic congenital heart disease. After the repair of this heart disease, patients reach adulthood with good quality of life due to technological advancement and better surveillance. However, in the long term, some anatomic and pathophysiological conditions are present and provide morbidity. Pulmonary regurgitation is the most common of them and leads to right ventricular dilation and dysfunction. Also, this is the cause of abnormal interventricular interactions and intrinsecal myocardial changes that lead to arrhythmogenic foci (reentrant circuits). The late complications result in global heart failure or sudden cardiac death (ventricular tachycardia). Dilation of the aortic root and ascending aorta are present due to the increased hemodynamic stress on the aortic wall and histological changes that decrease wall stiffness. This structural defect results in aortic regurgitation and contributes to heart failure. The cardiac magnetic resonance is the gold standard diagnostic procedure for cardiac morphologic and functional assessment. The implantable cardioverter-defibrillators are useful for the prevention of sudden death. The treatment of heart failure should be the most adjusted and the replacement of the pulmonary valve before cardiac irreversible dysfuntion is crucial.
