Abstract

Estamos perante uma “revolução etária”. A par das baixas taxas de natalidade aumenta o número de idosos, contribuindo para uma sociedade cada vez mais envelhecida. As alterações a nível cardiovascular verificadas no decurso do processo de envelhecimento transformam a hipertensão arterial (HTA), doença silenciosa, indolor e de lenta evolução, numa das mais frequentes co-morbilidades desta faixa etária. Com a introdução dos métodos de determinação da pressão arterial (PA) em ambulatório – auto-medição da PA (AMPA) e monitorização ambulatória da PA (MAPA), em associação com a medição casual da PA no consultório médico, dois novos conceitos de HTA surgiram – HTA de Bata-branca e, mais recentemente, a HTA Mascarada (HTM). A HTM apresenta uma elevada prevalência entre a população idosa e está associada a maior risco cardiovascular e lesão de órgão-alvo comparativamente à população normotensa, pelo que se enfatiza a necessidade de fazer um diagnóstico precoce e tratamento adequado, com vista à melhoria da qualidade de vida da população afectada. Contudo, muitos aspectos estão ainda por esclarecer no que a esta nova entidade diz respeito.

An “Age Revolution” is in front of us. The low birth rates, together with the rise of the older population, make our society increasingly older. The cardiovascular changes occurring during the ageing process turn the Arterial Hypertension (AHT), a slow, silent and painless disease, into one of the most frequent comorbidities in this group stage. With the introduction of the ambulatory blood pressure (BP) determination methods – home BP monitoring (HBPM), and the ambulatory BP monitoring (ABPM), in association with the occasional determination of this value at a medical office, two brand new concepts arose: the White Coat Hypertension and, more recently, the Masked Hypertension (MHT). The MHT is highly prevalent among the elderly, and is linked to a greater cardiovascular risk and target-organ lesion, comparing this group with a normotensive population, therefore emphasizing the need to get an early diagnosis and develop an adequate treatment, in order to achieve a better quality of life in the affected group. Nevertheless, many aspects related to this new entity remain to be clarified.
