Abstract

Introdução: O hiperparatiroidismo secundário é o resultado da falência de um ou mais componentes dos mecanismos homeostáticos do cálcio. No contexto da “Chronic Kidney Disease Mineral and Bone-related Disorder” (CKD-MBD), o hiperparatiroidismo caracteriza-se, para além das alterações ósseas, por um envolvimento cutâneo, articular, vascular, valvular, pulmonar e por um grande conjunto de alterações hormonais e dos parâmetros bioquímicos. Caso clínico: É apresentado um caso de uma doente de 48 anos, caucasiana, com doença renal crónica terminal e na qual foram detectadas múltiplas lesões osteolíticas, na tomografia computorizada, com risco de fractura óssea e compromisso neurológico. A doente foi previamente submetida a uma ablação cirúrgica a lesão na calote craniana, apresentando histopatologia compatível com tumor de células gigantes. Com posterior avaliação clínica, bioquímica e imagiológica do quadro clínico, foi assumido o diagnóstico tardio de múltiplos tumores castanhos associados a um hiperparatiroidismo secundário prolongado. Conclusão: Em doentes com sinais clínicos e radiológicos de lesões osteolíticas, que apresentam hiperparatiroidismo ou doença renal crónica terminal, os tumores castanhos devem ser incluídos no diagnóstico diferencial.

Introduction: Secondary hyperparathyroidism is the result of failure in one or more components of the calcium homoeostatic mechanism. In the context of Chronic Kidney Disease Mineral and Bone-related Disorder (CKD-MBD), hyperparathyroidism is characterized, in addition to bone changes, by a cutaneous, articular, vascular, valvular and lung involvement. Relevant hormonal and biochemical changes are present in this pathology. Case Report: We report a case of a 48 years old caucasian woman with end-stage renal disease, in which multiple osteolytic lesions, with possible neurologic compromise, were detected in computed tomography. The patient was previously submitted to surgical ablation of a skull lesion, in which giant cell tumor was admitted. With posterior clinical, biochemical and imaging evaluation, late diagnosis of brown tumors in long term secondary hyperparathyroidism was admitted. Conclusion: In patients with clinical and imaging signs of osteolytic lesions, with end-stage renal disease or hyperparathyroidism, brown tumors must be included in differential diagnosis.
