Abstract

Papiloma Invertido é um tumor benigno das fossas nasais e seios perinasais que apesar de raro tem um comportamento biológico desafiante por ser localmente agressivo, ter uma taxa de recorrência elevada e estar associado a transformação maligna. A sua raridade e o seu comportamento motivaram a realização deste trabalho que tem como objectivo a revisão da literatura existente sobre o Papiloma Invertido. A sua etiologia não é totalmente conhecida, pensando-se que o Vírus do Papiloma Humano (HPV) possa estar implicado apesar das bases moleculares desta doença não estarem ainda esclarecidas. O diagnóstico é anatomopatológico e imagiológico, podendo utilizar-se a TC, a RM e endoscopia nasal. Esta revisão inclui a definição de Papiloma Invertido, a descrição de dados históricos do mesmo, da sua epidemiologia, histopatologia e etiologia. Serão também abordados o comportamento biológico, as bases moleculares e a apresentação clínica do Papiloma Invertido. Por último, o diagnóstico, estadiamento e tratamento serão também discutidos. O tratamento deste tipo de lesão permanece ainda um desafio clínico para os otorrinolaringologistas. Contudo há a certeza de que a escolha de primeira linha é a cirurgia e de que é necessário um follow-up de longa duração neste tipo de doentes.

Inverted Papilloma is a benign tumour that arises in the nasal cavity and paranasal sinuses, and that despite being rare, shows a challenging biologic behaviour due to: its local aggressiveness, a high recurrence rate, and its association with a malignant transformation. This study was motivated by the rarity of the disease and by its behaviour and it aims at reviewing the existing literature on the subject. Although the disease's etiology is not entirely known, it is thought that Human Papilloma Virus (HPV) is involved, despite the fact that the molecular basis of this disease have not been clarified yet. The diagnosis is anatomopathological and imaging-based, and CT scan, MRI and nasal endoscopy may be used. This review includes the definition of Inverted Papilloma, a description of its historic data, epidemiology, histopathology and etiology. Inverted Papilloma's biologic behaviour, molecular basis and clinical presentation are also approached. Finally, diagnosis, staging and treatment are also discussed. The treatment of this type of lesion still remains a clinical challenge to otolaryngologists. However, there is certainty about using surgery as a first choice and on the fact that a long-term follow-up is necessary for this kind of patients.
