Abstract

Dermatomyositis (DM) is a systemic autoimmune disorder and the most common of the inflammatory myopathies. It results from an humoral attack against the skin and muscle capillaries and small arterioles. It begins with C3 activation which leads to the formation of C5b-9 membrane attack complex (MAC). This complex is deposited in the endomysial vasculature. The capillaries are then destroyed and muscle fibers undergo microinfarction. DM often presents with characteristic cutaneous features followed by proximal muscle weakness. It’s association with neoplasms, autoimmune disorders and some drugs has been estabilished. We report the case of a 53-year-old woman who was diagnosed with dermatomyositis 3 years after begining treatment with orlistat. The disease remmited completly after discontinuing the drug. Later, breast cancer was diagnosed.This article intends to report the association DM / Orlistat and to analyse the association DM / neoplasm and the posible role of Orlistat and Simvastatin as protectors in breast cancer.

A Dermatomiosite (DM) é uma doença auto-imune sistémica, sendo a mais comum das miopatias inflamatórias. É o resultado dum ataque humoral contra os capilares e arteríolas da pele e músculos. Inicialmente há uma ativação de C3, que culmina na formação do Complexo de Ataque Membranolítico C5b-9. A deposição deste no endomísio resulta na lise dos capilares e consequente isquémia muscular. A DM manifesta-se através de alterações cutâneas típicas, a que depois se associa fraqueza muscular proximal3. Está documentada a sua associação com neoplasias, outras doenças auto-imunes e alguns fármacos. Apresenta-se o caso de uma mulher de 53 anos a quem foi diagnosticada DM 3 anos após o início de terapêutica com Orlistat, tendo a doença regredido totalmente com a suspensão do fármaco. Mais tarde foi diagnosticada neoplasia da mama. Com este artigo, pretende-se relatar a associação DM / Orlistat, bem como analisar a associação DM / neoplasia e o eventual papel protetor do Orlistat e da Sinvastatina na neoplasia da mama.
