O principal objetivo, clinicamente relevante, no tratamento da micose do aparelho ungueal consiste na erradicação do foco infecioso. A resolução incompleta da infeção resulta em elevadas taxas de recorrência, o que significa que a onicomicose se torna frequentemente uma condição crónica que retoma o estado ativo no intervalo de vários anos. A prioridade nos recentes desenvolvimentos da farmacoterapia da onicomicose foi atribuída à monoterapia tópica com o intuito da sua utilização em doentes que não toleram a medicação oral ou preferem um fármaco tópico e, com a vantagem adicional, de combiná-los com fármacos orais e/ou dispositivos médicos para expandir as possibilidades de tratamento atuais. No status quo, as opções terapêuticas da onicomicose incluem dois novos fármacos tópicos, Efinaconazol e Tavaborol, aprovados no ano 2014 pela entidade reguladora competente ʻFood and Drug Administrationʼ.
The main, clinically relevant, objective in treat a nail apparatus mycosis is to eradicate the infectious focus. Incomplete resolution of the infection results in a high rate of recurrence, which means onychomycosis often becomes a chronic condition that retakes an active state in the range of several years. The priority in recent developments of onychomycosis’ pharmacotherapy lies on topical monotherapies for use in patients who cannot take oral medication or prefer a topical drug and, as an additional advantage, to combine them with oral drugs and/or medical devices to expand the current treatment possibilities. In the status quo, the therapeutic options for onychomycosis include two novel topical drugs, Efinaconazole and Tavaborole, both received approval from the competent regulatory agency, the Food and Drug Administration, in 2014.
