Abstract

A meningite (doença invasiva) é um processo inflamatório que envolve as meninges, tanto as que protegem o encéfalo, como as que protegem a medula espinhal, sendo provocada por diversos agentes, como vírus, bactérias, fungos e parasitas. As meningites bacterianas são um grave problema de saúde pública e são provocadas principalmente pelas bactérias Haemophilus influenzae, Neisseria meningitidis, Streptococcus pneumoniae, Streptococcus agalactiae e Listeria monocytogenes sendo potencialmente graves pelo facto de poderem deixar sequelas ou até mesmo provocar elevadas taxas de mortalidade, afectando principalmente as crianças. Tal situação exige um diagnóstico muito criteriosos e assertivo e um tratamento adequado face ao resultado do diagnóstico. Antes de se ter a confirmação do diagnóstico deve-se aplicar uma terapêutica empírica com base na idade, peso do indivíduo, entre outros factores de forma a mitigar a evolução da inflamação. Após confirmação do diagnóstico será reanalisada e ajustada a terapêutica consoante a bactéria identificada. Com a introdução das vacinas, foi possível verificar uma enorme alteração na disseminação da meningite, tornando-a mais controlada. Contudo é necessário manter uma vigilância activa, sobre esta patologia e simultaneamente promover o desenvolvimento de novos antibióticos para mitigar a resistência das bactérias.

Meningitis (invasive disease) is an inflammatory process involving meninges, those that protect the brain and those that protect the spinal cord, being caused by several agents, such as viruses, bacteria, fungi and parasites. Bacterial meningitis is a serious public health problem and is mainly caused by the bacteria Haemophilus influenzae, Neisseria meningitidis, Streptococcus pneumoniae, Streptococcus agalactiae and Listeria monocytogenes. It is potentially serious because they can cause sequelae or even cause high mortality rates, affecting mainly children. Such a situation requires a very careful and assertive diagnosis and an appropriate treatment against the result of the diagnosis. Before confirming the diagnosis, empirical therapy should be applied based on the age, weight of the individual, among other factors in order to mitigate the evolution of the inflammation. After confirmation of the diagnosis, the therapy will be re-analyzed and adjusted according to the bacteria identified. With the introduction of the vaccines, it was possible to verify a huge change in the spread of meningitis, making it more controlled. However, it is necessary to keep an active surveillance on this pathology while simultaneously promoting the development of new antibiotics to mitigate the resistance of the bacteria.
