Abstract

No decorrer do estágio curricular foram vários os casos que passaram pela Clínica Oftalmológica das Antas. Porém, foi selecionado um doente com diagnóstico de cataratas que já tinha sido submetido a cirurgia refrativa para correção de miopia. Esta escolha teve por base o aumento com que casos semelhantes a este têm ocorrido e à dificuldade que os mesmos conferem na determinação da lente intra-ocular a implantar no olho após a extração das cataratas. O doente, senhor caucasiano de 64 anos, surgiu no consultório referindo notar hipovisão e moscas volantes. Estes sintomas foram confirmados durante a consulta e com recurso a exames complementares de diagnóstico. Como solução foi proposta a extração das cataratas utilizando a técnica de facoemulsificação e vitrectomia pars plana para retirar condensações vítreas causadoras das moscas volantes do olho esquerdo. Para realizar a cirurgia às cataratas o doente efetuou um exame de biometria de coerência ótica, por forma a definir a potência da lente a introduzir no olho. Como o doente tinha no seu historial uma cirurgia refrativa, este exame tornou-se mais complexo uma vez que a forma da córnea foi alterada pela intervenção. Algumas semanas após a cirurgia surgiu um edema macular cistoide no olho direito, confirmado pela tomografia de coerência ótica, que causou um deterioramento da visão. Este foi tratado com injeção de anti-inflamatório na conjuntiva do doente. A oportunidade de seguir este caso de perto, permitiu perceber a importância que os exames complementares de diagnóstico têm durante todo o procedimento pré e pós-operatório. É através deles que é possível detetar as mais diversas complicações que possam surgir e agir o mais rapidamente possível. Por isso é importante o acompanhamento do doente antes e após as cirurgias.

During the curricular intership, several clinical cases passed through the Clínica Oftalmológica das Antas. However we selected a patient with cataract diagnosis who had already undergone refractive myopia surgery. We choose this case due to the increase frequency of similar cases and the difficulty to determinate the intraocular lens to implant on them. The patient, a Caucasian man with 64 years, appeared in the doctor’s clinic referring noticing hypovision and flying flies. During the consultation these symptoms were confirmed by complementary diagnostic tests. As solution, it was proposed the extraction of cataracts using the phacoemulsification technique and the removal of the vitreous floaters, which promotes the flying flies on the left eye, using the pars plana vitrectomy technique. To perform the cataract surgery the patient underwent an optical coherence biometry examination to define the type of lens to be inserted in the eye. This examination became more complicated once the corneal shape was altered by the refractive surgery intervention. Captured by optical coherence tomography, a cystoid macular edema emerged in the right eye a few weeks after surgery causing deterioration of vision. This was treated with an anti-inflammatory injection in the patient’s conjunctiva. The opportunity to follow this case closely, allowed me to understand the importance of complementary diagnostic tests throughout the pre and postoperative procedures. Through them is possible detect the most diverse complications that may arise and act as soon as possible. Therefore it is crucial to follow up the patient before and after the surgeries.
