A última década no Centro Hospitalar da Cova da Beira, E.P.E.

Abstract

Introdução: A brucelose humana é das zoonoses mais frequentes a nível mundial (19). Em Portugal, os distritos de Guarda e Castelo Branco ocupam o quarto lugar quanto às regiões mais afectadas (7). A doença é transmitida através do contacto directo ou indirecto com animais infectados. Os principais meios de transmissão são a ingestão de alimentos não pasteurizados e o contacto ocupacional (42). Objectivos: Caracterizar a brucelose no Centro Hospitalar Cova da Beira-EPE (CHCB-EPE) quanto à população afectada, sintomatologia apresentada, diagnóstico e tratamento. Proceder à análise comparativa dos casos verificados na última década no CHCB-EPE com os registados no Instituto Nacional de Estatística (INE) e na Direcção-Geral da Saúde (DGS). Material e Método: Foram diagnosticados 55 casos de brucelose no CHCB-EPE na última década. Procedeu-se ao estudo retrospectivo dos respectivos processos clínicos, nomeadamente à análise das variáveis: idade, sexo, ano, mês, sintomatologia predominante, primeiro resultado do Teste de Wright e tratamento aplicado. Resultados: Da amostra constatou-se predominância do sexo masculino (2,4:1) com uma idade média de 43 anos. As manifestações revelaram-se inespecíficas, sendo a febre o sinal e sintoma mais frequente (53%). O envolvimento osteo-articular foi a focalização mais comum, ocorrendo em 69% dos doentes. A artralgia localizada manifestou-se mais nas crianças do que adultos, com diferenças estatisticamente significativas (FET = 0,011). Em 48% dos doentes, o primeiro Teste de Wright foi 1/320 ou 1/160. Em 9,6% o resultado revelou-se negativo. O tratamento de eleição foi a combinação de doxiciclina com rifampicina (35%). Os doentes foram hospitalizados, com a duração média de internamento de 13 dias. Conclusão: A brucelose é uma doença de declaração obrigatória não erradicada em Portugal. Quando comparados os dados deste estudo com dados disponibilizados pelo INE, é clara a extensa subnotificação da doença. Os grupos etários da amostra constituem fonte de preocupação, uma vez que 40% dos casos correspondem a crianças e idosos. Foram detectados grupos vulneráveis à infecção, designadamente alcoólicos e toxicodependentes, que podem representar um grupo subdiagnosticado. O rastreio dos familiares em coabitação é um importante passo para a erradicação da brucelose.

Introduction: Human brucellosis is one of the most frequent zoonosis in the world (19). In Portugal, Guarda and Castelo Branco districts occupy the fourth place as more affected regions (7). The disease is transmitted by direct or indirect contact with infected animals. Ingestion of non pasteurized aliments and occupational contact are the dominant means of transmission (42). Objective: Characterize brucellosis in Centro Hospitalar Cova da Beira-EPE (CHCBEPE) regarding the affected population, exhibited symptomatology, diagnosis and treatment. Proceed to the comparative analysis of last decade verified cases in CHCBEPE with those registered in Instituto Nacional de Estatistica (INE) and Direcção Geral de Saúde (DGS). Material and Method: 55 cases of brucellosis have been diagnosed in CHCB-EPE in the last decade. The retrospective study of clinic processes, namely the analysis of the following factors: age, sex, year, month, pre dominant symptomatology, first result of Wright Test and applied treatment. Results: From the sample, 2,4:1 were males with an average of 43 years old. Manifestations reveal unspecific, being fever the most frequent symptom and sign (53%). The osteoarticular involvement was the most common focalization, seen on 69% of patients. Located arthralgia was seen mostly on children, with statistically significant differences (FET=0,011). On 48% of the patients, the first Wright Test was 1/320 or 1/160. On 9,6% the result was negative. The most chosen treatment was the combination of doxyclyne with rifampicin (53%). The patients were hospitalized for an average period of 13 days. Conclusion: Brucellosis is a disease of obligatory declaration not eradicated in Portugal. When data of this study is compared with data given by INE, it’s obvious the large under notification of the disease. The age groups of the sample are source of concern, once 40% of cases correspond to children and old people. Vulnerable groups to infection were identified, including alcoholics and drug addicts, which may represent a group underdiagnosed. The tracing of family members in cohabitation is an important step towards the eradication of brucellosis.
