Trombose Venosa Cerebral

Abstract

A trombose venosa cerebral (TVC) é uma doença cerebrovascular rara, mas as suas taxas de incidência têm vindo a aumentar, quer graças a melhores métodos de diagnóstico quer ao maior conhecimento desta patologia. Embora possa ocorrer em qualquer idade, ao contrário do acidente vascular cerebral (AVC) arterial, a TVC é mais frequente numa faixa etária mais jovem, sendo particularmente frequente em mulheres jovens com fatores de risco como o uso de contracetivo hormonal oral, gravidez ou puerpério. É uma doença por vezes de difícil diagnóstico por ter uma apresentação clínica e imagiológica muito diversa, variando consoante a área cerebral afetada, o calibre da veia ocluída e a consequente gravidade das lesões. Porém, em geral, o prognóstico é considerado bom, principalmente quando tratada atempada e adequadamente, sendo relatadas altas taxas de recuperação completa. Esta monografia visa, através da consulta e comparação de artigos e livros publicados nos últimos 15 anos, fazer uma descrição holística e atualizada da TVC no contexto da sua fisiopatologia, epidemiologia, fatores de risco e etiológicos, apresentação clínica, diagnóstico, tratamento e prognóstico.

Cerebral venous thrombosis (CVT) is a rare cerebrovascular disease, however, it’s incidence has been rising, either due to better diagnostic methods or to the increasing knowledge of this disease. Although it may occur at any age, contrary to arterial stroke, CVT is more frequent at a younger age range, being particularly frequent in young women with risk factors such as use of oral hormonal contraceptive, pregnancy or puerperium. It’s a disease that is sometimes difficult to diagnose due to its clinical and imagiologic presentation being so diverse, because it varies according to the affected cerebral area, the caliber of the occluded vein and consequent severity of the lesions. However, generally, the prognosis is considered to be good, mainly when treated timely and properly, with high rates of complete recovery being reported. This monography aims, through consultation and comparison of articles and books published in the past 15 years, to make a holistic and updated description of CVT in the context of it’s physiopathology, epidemiology, risk factors and causes, clinical presentation, diagnosis, treatment and prognosis.
