Efeito da exposição a concentrações sub- inibitórias de ciprofloxacina em Aliarcobacter butzleri

Abstract

O género Aliarcobacter pertence à família Arcobacteraceae, onde a espécie Aliarcobacter butzleri está inserida. Esta espécie é um enteropatógeno emergente que tem sido considerada um risco moderado para a saúde humana pela Comissão Internacional de Especificações Microbiológicas de Alimentos. Adicionalmente, vários isolados resistentes a antibióticos têm sido identificados, incluindo a ciprofloxacina. Apesar disso, a forma como a resistência desta bactéria surge ainda não se encontra totalmente elucidada. Nos últimos anos, o efeito de concentrações sub-inibitórias de antibióticos no desenvolvimento de resistência bacteriana tem sido um objeto de estudo já que são estas que normalmente se encontram no meio ambiente e assim proporcionam um ambiente favorável para a seleção de bactérias resistentes. Assim, o objetivo deste trabalho foi avaliar os efeitos da exposição a concentrações inferiores à concentração mínima inibitória (CMI) de ciprofloxacina na suscetibilidade e virulência de A. butzleri. Para isso, procedeu-se à: (i) determinação da CMI de diversos antimicrobianos, entre eles antibióticos, metais pesados e biocidas em 6 estirpes de A. butzleri de diferentes origens; (ii) à evolução adaptativa da estirpe A. butzleri DQ40A1 quando exposta a concentrações sub-inibitórias de ciprofloxacina e determinação da frequência de mutação das populações evoluídas; (iii) avaliação da diversidade genética entre a estirpe nativa e populações evoluídas; (iv) avaliação do efeito da adaptação de A. butzleri em concentrações sub-inibitórias de ciprofloxacina na suscetibilidade e na virulência da bactéria. Após a determinação da CMI das estirpes de A. butzleri, todas mostraram ser suscetíveis à amoxicilina, eritromicina e ciprofloxacina, tendo a estirpe A. butzleri DQ40A1 sido selecionada para os ensaios subsequentes, devido ao seu perfil de resistência a antimicrobianos, por ter o seu genoma completamente sequenciado, e por fim por existirem estudos com o mesmo objetivo para outros antibióticos. Esta estirpe foi então exposta a uma evolução adaptativa laboratorial na ausência de antibiótico ou na presença de 1/2×, 1/4×, 1/8× e 1/16× CMI de ciprofloxacina, durante 12 dias. Ao avaliar a frequência de mutação das populações em estudo, a exposição a concentrações sub-inibitórias de ciprofloxacina parece induzir um aumento da frequência de mutação, no entanto, os resultados não se mostraram conclusivos. A análise da diversidade genética das várias populações obtidas ao final dos 12 dias de adaptação não demonstrou diferenças quando estas foram comparadas com a estirpe nativa através do uso de Enterobacterial Repetitive Intergenic Consensus Polymerase Chain Reaction (ERICPCR). Adicionalmente, as populações em estudo, não apresentaram uma diminuição da suscetibilidade à ciprofloxacina ou alteração na suscetibilidade aos restantes antimicrobianos testados (amoxicilina, eritromicina, zinco, cádmio ou clorohexidina), nem na capacidade de cada população formar biofilmes, nem alterações significativas na motilidade das populações adaptadas às concentrações sub-inibitórias de ciprofloxacina. Assim, de acordo com os resultados obtidos foi possível verificar que as concentrações sub-inibitórias de ciprofloxacina não parecem afetar a suscetibilidade e a virulência bacteriana no intervalo e condições testadas. Contudo, mais estudos são necessários para tirar conclusões efetivas acerca deste comportamento.

The genus Aliarcobacter belongs to the family Arcobacteraceae, which includes the species Aliarcobacter butzleri. This species is an emerging enteropathogen that has been considered a moderate risk to human health by the International Commission on Microbiological Specifications for Foods. Additionally, several antibiotic-resistant isolates have been identified, including those resistant to ciprofloxacin. Despite this, the way in which this bacterium’s resistance arises has not yet been fully elucidated.In recent years, the effect of sub-inhibitory concentrations of antibiotics in the development of bacterial resistance has become a focus of study, as these are typically found in the environment and thus provide a favourable environment for the selection of resistant bacteria. Therefore, the objectives of this study were to evaluate the effects of exposure to concentrations below the minimum inhibitory concentrations (MIC) of ciprofloxacin on the susceptibility and virulence of A. butzleri. To achieve this, we conducted to the following: (i) determination of the MIC of various antimicrobials, including antibiotics, heavy metals and biocides, for six A. butzleri strains of different origins; (ii) the adaptive evolution of A. butzleri DQ40A1 strain when exposed to sub-inhibitory concentrations of ciprofloxacin and determination of the mutation frequency of the evolved populations; (iii) assessment of genetic diversity among the native strain and evolved populations; (iv) evaluation of the effect of A. butzleri’ adaptation in sub-inhibitory concentrations of ciprofloxacin to the susceptibility and virulence of the bacterium. After determining the MIC of each A. butzleri’ strain, all were susceptible to amoxicillin, erythromycin and ciprofloxacin, with the A. butzleri DQ40A1 strain being selected for subsequent tests. This strain was then exposed to laboratory adaptive evolution in the absence of antibiotic or in the presence of 1/2×, 1/4×, 1/8× and 1/16× MIC of ciprofloxacin for 12 days. When evaluating, the mutation frequency of populations under study the exposure to subinhibitory concentrations of ciprofloxacin may appear to induce an increase in the mutation frequency, however, the results do not prove to be conclusive. The genetic diversity analysis of the various populations obtained at the end of 12 days of adaptation showed no differences when compared with the native strain using Enterobacterial Repetitive Intergenic Consensus Polymerase Chain Reaction (ERIC-PCR). Additionally, the populations under study did not show a decrease in susceptibility to ciprofloxacin or change in susceptibility to the remaining antimicrobials tested (amoxicillin, erythromycin, zinc, cadmium or chlorhexidine), nor in the ability of each population to form biofilms nor in the motility of the three populations analyzed. Thus, according to the results obtained, it was possible to verify that the sub-inhibitory concentrations of ciprofloxacin do not seem to affect bacterial susceptibility or virulence in the range and conditions tested. However, more studies are needed to draw effective conclusions about this behavior.
