Abstract

Introduction: Hernioplasty has exhibited a progressive performance escalation in the clinical landscape of General Surgery, propelled by advancements in surgical techniques and demographic shifts characterized by aging and obesity. Abdominoplasty, driven by aesthetic aspirations for the abdominal wall, is concurrently implemented with hernioplasty to synergistically address health promotion and aesthetic considerations, ultimately augmenting patient’s quality of life. Objectives: This study aims to delineate hernioplasty and abdominoplasty as isolated surgical procedures, while concurrently elucidating the intricacies of their combined application. Emphasis is placed on detailing indications, advantages, contraindications, and nuanced variations in the concurrent execution of both techniques. Methods: Methodological protocols center on utilizing PubMed as a search engine and consulting Yuri W. Novitsky’s "Hernia Surgery" for literature review. Search criteria spanned three distinct periods, with a focus on English-language articles and adherence to predefined keywords. Development: Simultaneous abdominoplasty with hernioplasty is deployed in individuals seeking simultaneous improvement in abdominal aesthetics and correction of abdominal wall defects. This approach is also considered for patients with abdominal hernias and surplus skin and tissues that may complicate surgery. The combined procedure, whether through laparoscopic or open techniques, necessitates meticulous adjustments to mitigate potential impacts on surgical outcomes. Simultaneous execution provides heightened exposure of the abdominal wall for reconstruction, yielding aesthetic enhancements and mitigating long-term hernia recurrence risk. However, the excision of substantial tissue quantities inherently introduces an augmented postoperative complication profile, offset by the noted satisfaction among patients undergoing concurrent abdominoplasty with hernioplasty. Conclusion: In the simultaneous execution of hernioplasty with abdominoplasty, a comprehensive exploration of all facets related to excess tissue excision during abdominal hernia repair is imperative, duly considering the holistic well-being of the patient.

Introdução: A hernioplastia apresenta um crescente desempenho na prática clínica de Cirurgia Geral, motivada tanto pela evolução das técnicas cirúrgica como pelo envelhecimento e obesidade da população. A abdominoplastia surge da vontade da melhoria estética da parede abdominal. A prática simultânea das técnicas parte de forma a integrar a promoção de saúde e estética, melhorando a qualidade de vida dos pacientes. Objetivos: Os objetivos do trabalho são descrever a hernioplastia e abdominoplastia como técnicas isoladas, assim como descrever a sua prática simultânea, enfatizando as indicações, vantagens, contraindicações e alterações na realização concomitante de ambas as técnicas. Métodos: A metodologia da monografia centrou-se na utilização do PubMed, como motor de busca, e na consulta do livro Hernia Surgery de Yuri W. Novitsky. Foram realizadas pesquisas em três períodos diferentes, cujos critérios residiam na utilização de artigos em inglês e na referência das palavras-chave adotadas no trabalho. Desenvolvimento: A abdominoplastia simultânea a hernioplastia surge em pacientes que buscam aprimorar a estética abdominal e com defeitos da parede abdominal, sendo também considerada em pacientes com hérnias abdominais e excesso de pele e tecidos que poderiam complicar a cirurgia. Esta abordagem pode ser realizada tanto pela combinação da abdominoplastia com hernioplastia por via laparoscópica quanto com hernioplastia por via aberta, mas devem ser ajustadas para prevenir impacto nos resultados cirúrgicos. A prática simultânea das técnicas proporciona maior exposição da parede abdominal para reconstrução, resultando em melhorias estéticas e reduzindo o risco de recorrência de hérnia a longo prazo. No entanto, a excisão de grandes quantidades de tecido impõe um aumento inerente de complicações no pós-operatório, contrabalançando com a alta satisfação dos pacientes submetidos a abdominoplastia simultânea a hernioplastia. Conclusão: Na realização concomitante da hernioplastia com abdominoplastia devem ser explorados todos os aspetos da excisão do excesso de tecido durante a reparação das hérnias abdominais, assim como a repercussão no bem-estar completo do paciente.
