Abstract

Introdução: A puberdade representa um evento fisiológico no qual ocorre a aquisição da capacidade reprodutiva e o completo desenvolvimento dos órgãos sexuais e físicos. Existem diversos distúrbios relacionados com o seu normal decorrer, nomeadamente a puberdade precoce e o atraso pubertário. O aumento da incidência da puberdade precoce central em conjunto com o aumento da prevalência da obesidade infantil levou à colocação da hipótese de uma possível interação. Esta interação tem sido amplamente investigada de modo, a tentar clarifica-la. Metodologia: Para a elaboração deste trabalho foi realizada uma pesquisa online, na base de dados PubMed®. Foram incluídos artigos que estudavam a associação da puberdade precoce central com a obesidade infantil, nos idiomas inglês, espanhol e português, publicados desde 2010. Foram ainda, consultados manuais e publicações de entidades de referência. Resultados: Os resultados da pesquisa realizada indicam que há uma relação entre puberdade precoce central e obesidade infantil, sugerindo que a obesidade pode influenciar e possivelmente antecipar o aparecimento da puberdade precoce central. Os mecanismos fisiopatológicos dessa relação são complexos e multifatoriais, envolvendo uma interação entre os sistemas metabólico e endócrino e envolvendo diversas proteínas, vias metabólicas e hormonas. Exemplos incluem a leptina, grelina, kisspeptinas, vias mTOR e AMPK, insulina e ceramidas. Conclusão: Em suma, a evidência aponta para uma associação entre a puberdade precoce central e a obesidade, destacando a importância de uma abordagem integrada na compreensão e gestão dessas condições.

Introduction: Puberty represents a physiological event in which the acquisition of reproductive capacity and the complete development of the sexual and physical organs occurs. There are several disorders that may affect its normal course, namely precocious puberty and pubertal delay. The increasing incidence of central precocious puberty has been associated with the rising of childhood obesity prevalence. This interaction has been widely investigated. Methodology: For this study, an online search was carried out in the PubMed® database. Articles were included with following conditions: studied the association between central precocious puberty and childhood obesity, in English, Spanish and Portuguese, published since 2010. Manuals and publications of reference organisations were also consulted. Results: The results of the research carried out indicate that there is a relationship between central precocious puberty and childhood obesity, suggesting that obesity can influence and possibly anticipate the onset of central precocious puberty. The pathophysiological mechanisms of this relationship are complex and multifactorial, involving an interaction between the metabolic and endocrine systems and involving various proteins, metabolic pathways and hormones. Examples include leptin, ghrelin, kisspeptins, mTOR and AMPK pathways, insulin and ceramides. Conclusion: In summary, the evidence points to an association between central precocious puberty and obesity, highlighting the importance of an integrated approach to understanding and managing these conditions.
