Abstract

Introdução: Considera-se infertilidade a incapacidade de obter uma gravidez após um período superior a um ano de relações sexuais regulares e desprotegidas, isto é, sem utilização de métodos contracetivos (OMS). A disfunção sexual masculina compreende perturbações do desejo sexual, da excitação, do orgasmo ou dor. Não raramente, essas duas condições manifestam-se no mesmo indivíduo, podendo existir uma relação de causalidade entre si. O presente projeto propõe-se a relacionar disfunção sexual e infertilidade, no elemento do sexo masculino de casais heterossexuais que recorram a Reprodução Medicamente Assistida (RMA). Métodos: Pesquisa de artigos no motor de busca Pubmed, em língua inglesa e portuguesa, com preferência a artigos publicados nos últimos 20 anos, mas sem restrições de data de publicação, utilizando múltiplas associações entre os termos “male”, “infertility”, “sexual disfunction”. Resultados: A interpretação da bibliografia demonstrou que a disfunção sexual masculina é mais prevalente nos homens com dificuldades reprodutivas. Aquando do diagnóstico de infertilidade, os casais experienciam um vasto leque de emoções negativas que poderão ter impacto na sua sexualidade e performance sexual. Dessa forma, a disfunção sexual pode, não apenas ser consequência da infertilidade, mas também causa. Conclusão: Perante os resultados obtidos, conclui-se que a abordagem do homem com dificuldades reprodutivas deve ser completa, incluindo um exame físico minucioso, uma anamnese e história sexual e reprodutiva cuidadas, e ainda uma abordagem psicológica, com o auxílio de uma equipa médica variada que inclua psicoterapeuta e terapeuta sexual.

Introduction: Infertility is considered the inability to obtain a pregnancy after a period longer than one year of regular and unprotected sexual intercourse, that is, without the use of contraceptive methods (WHO). Male sexual dysfunction comprises disturbances in sexual desire, arousal, orgasm or pain. Not rarely, these two conditions manifest themselves in the same individual and a causal relationship may exist between them. This project aims to relate sexual dysfunction and infertility in males of heterosexual couples using Medically Assisted Reproduction (MAS). Methods: Search of articles in the Pubmed search engine, in English and Portuguese language, with preference to articles published in the last 20 years, but without restrictions on publication date, using multiple associations between the terms "male", "infertility", "sexual dysfunction". Results: Interpretation of the literature showed that male sexual dysfunction is more prevalent in men with reproductive difficulties. When infertility is diagnosed, couples experience a wide range of negative emotions that may impact their sexuality and sexual performance. Thus, sexual dysfunction may not only be a consequence of infertility, but also its cause. Conclusion: In view of the results obtained, we conclude that the approach to men with reproductive difficulties must be complete, including a thorough physical examination, a careful anamnesis and sexual and reproductive history, and also a psychological approach, with the help of a varied medical team that includes a psychotherapist and a sex therapist.
