O impacto do jejum no prognóstico do cancro

Abstract

No mundo inteiro, milhões de pessoas vivem com o diagnóstico de cancro. Nos países desenvolvidos, a incidência e a prevalência desta doença tem vindo a aumentar devido a uma população mais envelhecida e contínua exposição a fatores de risco. Em Portugal, no ano de 2020, registaram-se cerca de 60467 novos casos e 30168 de mortes causadas por esta doença crónica. Embora a alimentação / restrição calórica já provaram ser ferramentas eficazes na prevenção da incidência de cancro, pouco se sabe do seu uso no prognóstico da doença. Recentemente, surgiram evidências científicas que relacionam práticas do jejum com o aumento da eficácia de terapias convencionais como a quimioterapia, radioterapia e imunoterapia. Assim, estudos contemporâneos equacionam o potencial do jejum como terapia adjuvante, uma vez que o seu impacto diferencial na resistência ao stress em células saudáveis vs tumorais começa a suscitar interesse na comunidade científica. Após a revisão bibliográfica sobre o tema, pode observar-se que o jejum pode ter vários efeitos benéficos no tratamento anti-cancerígeno, em termos de qualidade de vida e de toxicidade dos fármacos. Outro aspeto positivo desta intervenção alimentar, é que ajuda a promover a participação dos doentes no processo terapêutico, algo que é fortemente recomendado pela comunidade oncológica. Conclui-se que para validar estes estudos primordiais são necessários mais ensaios clínicos e com uma amostra estatisticamente mais relevante. De maneira a ser possível inferir sobre os resultados, é de salientar que as variáveis em teste devem ser reduzidas ao mínimo, de maneira a isolar os efeitos de cada combinação entre tipo de cancro vs estadio vs terapia vs tipo de jejum e a sua duração.

World wide, millions of people live with a cancer diagnosis. In developed countries, the incidence and prevalence of this disease has been increasing due to an aging population and continued exposure to risk factors. In Portugal, in the year 2020, there were about 60467 new cases and 30168 deaths caused by this chronic disease. Although diet/caloric restriction have proven to be effective tools in preventing cancer incidence, little is known about their use in the prognosis of the disease. Recently, scientific evidence has emerged that links fasting practices with increased efficacy of conventional therapies such as chemotherapy, radiation and immunotherapy. After reviewing the literature on the subject, it can be seen that it can have several beneficial effects on anticancer treatment, in terms of quality of life and drug toxicity. Another positive aspect of this dietary intervention is that it helps to promote patient participation in the therapeutic process, something that is strongly recommended by the oncology community. It is concluded that to validate these primary studies, more clinical trials are needed and with a more statistically relevant sample size. In order to be able to make any conclusions about the results, it should be noted that the variables under test should be kept to a minimum, in order to isolate the effects of each combination between cancer type vs stage vs therapy vs type or duration of fasting.
