Abstract

A diverse clinical practice was employed through the course of a six-month internship, with the aim of concluding a Master’s degree in Optometry and Vision Sciences, from which were gathered three specific case studies that were analysed and the adequate treatments prescribed. The mentioned case studies are suspicion of Cystoid Macular Edema (ECM), divergence insufficiency and multifocal contact lenses adaptation. When it came to the pathology case, the suspicion of ECM was detected due to the diminished visual acuity with no apparent justification, which led to the need of carrying out additional examinations. In this case, the proposed course of action was a referral of the patient to an ophthalmologist. In the case of divergence insufficiency, related to binocular vision, despite being a benign condition, it still caused great discomfort to the patient. With this in mind, new prescription lenses and the addition of prisms were advised as a suitable treatment. Finally, when it came to the adaptation to multifocal contact lenses, a need to change the lenses’ material aroused, due to the physiological and visual characteristics of the patient, as well as the implementation of a technique that would allow for the testing of the visual acuity at all distances, facilitating the process of choosing the correct prescription lenses.

Verificou-se uma prática clínica diversificada ao longo de seis meses de estágio com intuito de finalização de mestrado, onde, no presente trabalho se reúnem três casos específicos observados e o tratamento indicado. Os casos apresentados são: suspeita de edema macular cistoide, insuficiência de divergência e adaptação de lentes de contacto multifocais. Relativamente ao caso da patologia, a suspeita de edema macular cistóide foi detetada através de baixa de acuidade visual sem motivo aparente, levando à necessidade de realizar exames complementares. Neste caso, o curso de ação mais indicado foi o reencaminhamento do paciente para um oftalmologista. No caso da insuficiência de divergência, relacionada com a visão binocular, apesar de ser benigna, apresenta notável incomodo para o paciente. Desta forma, o tratamento mais indicado foi a alteração da graduação e a adição de prismas. Por último, o caso de adaptação de lentes de contacto multifocais, foi necessário alterar os materiais das mesmas devido às necessidades de visão e fisiológicas do paciente, e ainda a utilização de uma técnica que permite observar a acuidade visual a todas as distâncias, facilitando assim a obtenção da graduação mais indicada.
