Fatores que influenciam o sucesso da terapia pulpar vital

Abstract

Objetivo: Identificar e avaliar os fatores que influenciam a taxa de sucesso da terapia pulpar vital (TPV). Materiais/Métodos: Foi realizada uma pesquisa bibliográfica nas bases de dados PubMed, ResearchGate, Cochrane e Google Scholar. Na pesquisa, foram feitas permutações entre as seguintes palavras-chave: permanent teeth, primary teeth, vital pulp therapy, reversible pulpitis, irreversible pulpitis, direct pulp capping, indirect pulp capping, pulpotomy, partial pulpotomy, capping materials. De um total de 442 artigos, foram selecionados 72 escritos em inglês, entre 1978 e 2024, cujos resumos se adequavam ao objetivo da revisão. Excluindo revisões de literatura e meta-análises, foi elaborada uma tabela principal com 64 artigos. A partir desta, foram criadas 14 tabelas adicionais, divididas por temas específicos: dentes decíduos ou permanentes, técnica usada e diagnóstico pulpar. Resultados: A revisão descreve vários conceitos que contribuem para a preservação e sucesso da vitalidade do tecido pulpar em dentes decíduos ou permanentes. A literatura não é consensual quanto ao facto da taxa de sucesso da TPV poder ser influenciada por fatores como a idade do paciente, a estrutura remanescente do dente, o material usado para a TPV e o diagnóstico pulpar. Materiais como agregado trióxido mineral (MTA) e hidróxido de cálcio (HC) são frequentemente mencionados devido à sua eficácia na preservação da vitalidade pulpar. Observou-se que a TPV tem uma alta taxa de sucesso em dentes com pulpites reversíveis e também pode ser efetiva em casos de pulpites irreversíveis e necrose pulpar, dependendo da técnica e do material utilizado. Conclusão: Fatores como a idade, estrutura remanescente do dente, material usado para a TPV e diagnóstico pulpar parecem influenciar a taxa de sucesso da TPV. Em contrapartida, fatores como o sexo do paciente, tipo de dente ou material restaurador usado não parecem ter impacto significativo na taxa de sucesso da TPV.

Objective: To identify and evaluate the factors influencing the success rate of vital pulp therapy (VPT). Materials/Methods: A bibliographic search was conducted in PubMed’s, ResearchGate’s, Cochrane’s, and Google Scholar’s databases. The search included permutations of the following keywords: permanent teeth, primary teeth, vital pulp therapy, reversible pulpitis, irreversible pulpitis, direct pulp capping, indirect pulp capping, pulpotomy, partial pulpotomy, capping materials. Out of 442 articles, 72 written in English between 1978 and 2024 were selected, based on the abstract fitting the review's objective. Excluding literature reviews and meta-analyses, a main table with 64 articles was created. From this main table, 14 additional tables were created, categorized by specific themes: primary or permanent teeth, technique used, and pulp diagnosis. Results: The review describes several concepts that contribute to the preservation and success of pulp tissue vitality in primary or permanent teeth. The literature is not consensual on whether the success rate of VPT can be influenced by factors such as the patient's age, the remaining tooth structure, the material used for VPT, and the pulp diagnosis. Materials like Mineral Trioxide Aggregate (MTA) and Calcium Hydroxide (CH) are frequently mentioned due to their effectiveness in preserving pulp vitality. It was observed that VPT has a high success rate in teeth with reversible pulpitis and can also be effective in cases of irreversible pulpitis and pulp necrosis, depending on the technique and material used. Conclusion: Factors such as age, remaining tooth structure, material used for VPT, and pulp diagnosis seem to influence the success rate of VPT. On the other hand, factors like patient gender, type of tooth, or restorative material used do not appear to have a significant impact on the success rate of VPT.
