Abstract

ABSTRACT - Subjectivity on animal pain assessment - Pain is one of the most important aspects concerning animal welfare, having the 21st century Veterinarian, the ethical responsibility to avoid inflicting pain on the animal or try to make the pain minimal. For pain recognition, the veterinarian finds in literature a set of behaviors and reactions, which may be indicators of pain in the animal, as well as a set of pain scales, that may help him in that task. In spite of this, it remains a challenge since there is no direct communication between the patient and the doctor, being the interpretation of these behaviors dependent on the subjectivity of the observer. The study has been performed in thirteen (13) bitches submitted to an ovariohysterectomy and had as main goals to evaluate the influence of the academic degree and the experience level of the operator in the determination of the animal pain degree, comparing the results according to different kinds of pains scales. The evaluators were split in two groups: Interns (same level of clinical experience) and Nurses (Experience nurse vs Inexperienced Nurse). The evaluation of animal pain was carried out during the postoperative period (2, 4, 6, 8 and 24 hours after extubation) by two interns and two nurses at each time, with the aid of different scales, providing descriptive scales to the interns group - Glasgow Composite Pain Scale (short form) and Pain Scale of Melbourne University – while to the nursing group semiobjective scales were provided – Visual Analogic Scale and Pain Scale from the Colorado University. The conclusions were that the semi-objective scales show a greater variability between evaluators, but are simpler and faster to apply on clinical practice when compared to the descriptive scales and the experience factor decreases the difference in the results obtained between semi-objective scales and descriptive scales.

A dor é um aspetos dos mais importantes no que diz respeito ao bem-estar animal, tendo o Médico Veterinário do século XXI a responsabilidade ética, para com o seu paciente, de a evitar ou minimizar. Para o seu reconhecimento o Médico Veterinário encontra na literatura um conjunto de comportamentos e reações que podem ser indicadores de dor no animal, assim como um conjunto de escalas de avaliação de dor que o podem auxiliar nessa tarefa. Apesar disso, continua a ser um desafio uma vez que não há comunicação direta entre o paciente e o médico, estando a interpretação desses comportamentos sujeitos à subjectividade do observador. O estudo foi realizado em 13 cadelas submetidas a ovariohisterectomia e teve como principais objetivos avaliar a influência do grau académico e nível de experiência do operador na determinação do grau de dor do animal, comparando os resultados obtidos segundo as diferentes escalas de dor utilizadas. Os avaliadores foram divididos em 2 grupos: Estagiários (nível de experiência clínica semelhante) e Enfermagem (Enfermeira experiente versus Enfermeira inexperiente). A avaliação da dor animal realizou-se no período pós-operatório (2, 4, 6, 8, 24 h após extubação) por 2 estagiários e 2 enfermeiras em cada tempo, com o auxílio de diferentes escalas, sendo que ao grupo de estagiários foram fornecidas escalas descritivas – Escala Composta de Dor de Glasgow (forma abreviada) e Escala de Dor da Universidade de Melbourne – e ao grupo de enfermagem foram fornecidas escalas semi-objetivas – Escala Visual Analógica e Escala de Dor adaptada da Universidade de Colorado. Concluiu-se que as escalas semi-objetivas apresentam uma maior variabilidade entre avaliadores mas são de aplicação mais simples e rápida na prática clínica quando comparadas com as escalas descritivas e que o fator experiência diminui a diferença entre escalas semi-objetivas e escalas descritivas.
