Abstract

A ovariohisterectomia é um dos procedimentos cirúrgicos mais frequentes na prática veterinária por ser o meio mais eficaz no controlo da população animal. Em gatas, é recomendado realizá-la por volta dos 6 meses de idade, sendo aconselhável sempre antes da puberdade. Este estudo teve como objectivo a comparação entre dois procedimentos cirúrgicos para a ovariohisterectomia: aproximação pelo flanco e pela linha média. Foram realizadas 24 ovariohisterectomias, 13 com aproximação pelo flanco e 11 pela linha média. As gatas eram saudáveis, tinham entre 4 e 24 meses e não se encontravam em estro, lactação ou gestação. A duração de cada período da cirurgia foi registada, assim como o tamanho final da incisão e as complicações intra e pós-operatórias. De acordo com a duração total de ambos os procedimentos, a aproximação pelo flanco foi significativamente mais rápida (p= 0,012). A única parte da cirurgia em que o tempo de duração foi semelhante entre as duas abordagens, foi na exteriorização e sutura do corpo uterino. Em todos os outros períodos, a aproximação pela linha média foi significativamente mais demorada. Relativamente ao tamanho final médio da incisão, no flanco foi significativamente mais curto (p= 0,001). A grande desvantagem da aproximação pelo flanco foi a fraca visibilidade do interior da cavidade abdominal, com consequente aumento das complicações intra-operatórias, tendo sido registado um caso de hemorragia por perda de um pedículo ovárico. Para além disso, a incidência de complicações pós-operatórias foi mais elevada nesta abordagem, onde se detectaram 4 casos. Na linha média houve registo de apenas uma gata com complicação pós-operatória.

ABSTRACT - COMPARATIVE STUDY OF FELINE OVARIOHISTERECTOMY BY THE FLANK AND MIDLINE APPROACHES - Ovariohysterectomy is one of the most frequently performed surgical procedures in veterinary practice in order to control pet population. In cats the recommended age to perform ovariohysterectomy is 6 months and it’s always advisable before the puberty. The objective of this study was to compare two different approaches for ovariohysterectomy: by the flank and by the midline. Twenty-four ovariohisterectomies were performed, 13 by the flank approach and 11 by the midline. The cats were healthy, with 4 to 24 months and were not in oestrus, lactating or pregnant. The duration of each part of the procedures was recorded, as well as the final length of the incision and the intra and postoperative complications. The total duration of the surgery was significantly lower in the flank approach (p= 0,012). The only part of the surgery where the time was similar between the two approaches was during exteriorization and suture of the uterine body. In all the other times the midline approach took significantly longer. The mean final length of the flank incisions was significantly shorter (p= 0.001). The great disadvantage of the flank approach is the lack of visibility of the abdominal cavity, which consequently increases the intraoperative complications. It was recorded a case of hemorrhage for loss of the ovarian pedicle before it was sutured. Additionally, there was a higher incidence of postoperative complications in this approach, detected in 4 cases. In the midline approach, only one cat had postoperative complication.
