Abstract

Atualmente observa-se uma crescente procura por serviços cirúrgicos, sendo uma das suas principais causas o envelhecimento da população. Deste modo, torna-se importante que os gestores antecipem esta procura e desenvolvam ferramentas que permitam melhorar os níveis de eficiência destes serviços. A atividade cirúrgica gera a maior parte do financiamento hospitalar, mas também constitui a maior parte dos seus custos. O Bloco Operatório (BO) consome parte significativa dos recursos financeiros da atividade hospitalar e da atividade cirúrgica em especial. Muitos estudos referem que na base do aumento dos custos hospitalares está a ineficiência da gestão das instituições de saúde. Torna-se assim fundamental o desenvolvimento de procedimentos adequados de planeamento cirúrgico. O presente projeto foca-se numa abordagem prática ao planeamento de cirurgias. Pretende-se satisfazer a procura dos pacientes por procedimentos cirúrgicos, mas também melhorar a organização do BO e da atividade cirúrgica, respeitando os limites definidos pelos recursos. Para o problema em estudo utilizam-se dados da empresa Luísa Magalhães Ramos - Cirurgia Plástica, Lda, uma clínica de cirurgia plástica onde o planeamento cirúrgico se efetua manualmente. Estão disponíveis quatro médicas para realizar cirurgias, sendo que na clínica se fazem 24 procedimentos cirúrgicos. Com base nos dados de 2022, prevê-se a procura mensal de cirurgias por procedimento em 2023. Posteriormente, desenvolvem-se modelos de Programação Linear Inteira cujo objetivo é maximizar o lucro. Estes modelos permitem identificar o serviço mensal de cada médica, tendo em conta o tempo máximo disponível que estas têm para trabalhar. Os modelos são resolvidos recorrendo ao OpenSolver do Excel. Foram implementados em Visual Basic for Applications (VBA), permitindo que a solução seja de fácil obtenção e interpretação por todos os utilizadores.

Currently, there is a growing demand for surgical services, with one of the main causes being the aging population. Therefore, it is important for managers to anticipate this demand and develop tools that can improve the levels of efficiency of these services. Surgical activity generates the majority of hospital funding, but it also constitutes the majority of its costs. The Operating Room consumes a significant portion of the financial resources of hospital activities and of surgical activities. Many studies indicate that the inefficiency of healthcare institution management is at the root of increasing hospital costs. The development of appropriate surgical planning procedures is then essential. The current project focuses on a practical approach to surgical planning. Its aim is to meet patients' demand for surgical procedures, while also enhancing the organization of the Operating Room (OR) and surgical activities, considering the available resources. The problem under study uses data from the company Luísa Magalhães Ramos - Plastic Surgery, Lda, a plastic surgery clinic where surgical planning is done manually. There are four surgeons available to perform surgeries and the clinic offers 24 surgical procedures. Based on the 2022 data, the monthly demand for surgeries per procedure in 2023 is forecasted. Subsequently, Integer Linear Programming models are developed, with the goal of maximizing profit. These models enable the identification of each surgeon's monthly service, taking into account their maximum available working time. Models are solved using Excel's OpenSolver. They have been implemented in Visual Basic for Applications (VBA), making the solution easily obtainable and interpretable by all users.
