Abstract

A classe dos tumores da pele e tecidos moles abrange um vasto leque de neoplasias, entre as quais se encontram as mais comuns na medicina veterinária. Os tumores mesenquimatosos estão inseridos nesta classe, e compreendem, entre outros, o fibrossarcoma. Esta neoplasia é considerada a mais frequente da pele e tecido subcutâneo, sendo comum no gato. Existem várias etiologias, como a infecção pelos vírus da leucemia felina e do sarcoma felino, traumatismo, administração de vários injectáveis, implantação de cápsula de identificação electrónica e retenção de material cirúrgico. A apresentação clínica do fibrossarcoma é variável, dependendo da espécie, idade e etiologia. Este tumor pode apresentar-se como uma massa de dimensão variável, de consistência mole a firme, e sem verdadeira cápsula. É localmente invasivo mas a sua capacidade de metastizar é baixa, encontrando-se associado a uma frequência alta de recidivas. O tratamento cirúrgico é desafiante, e a sua complementação com várias terapêuticas como a radioterapia, quimioterapia e imunoterapia, já foi estudada. Este estudo retrospectivo apresenta a caracterização de uma amostra de 34 casos de fibrossarcoma felino que foram seguidos no Hospital Veterinário do Restelo, assim como a análise estatística das variáveis associadas à mortalidade, tempo de sobrevida e recidivas. Neste estudo não houve predisposição de sexo ou raça. A idade média foi 9 (±3) anos. A infecção pelos vírus da leucemia felina e do sarcoma felino não foi confirmada em nenhum caso. A etiologia suspeita dos casos de fibrossarcoma no local de injecção foi a vacinação. Apenas 2 gatos apresentaram metástases, mas a recidiva ocorreu em 65% dos casos. As variáveis associadas à mortalidade e ao tempo de sobrevida da totalidade de casos foram: a idade no primeiro diagnóstico, sexo, localização anatómica, dimensão do tumor, ocorrência de recidiva e número de cirurgias. Estas análises foram ainda aplicadas aos casos de fibrossarcoma no local de injecção, tendo em conta a sua importância clínica. Foi investigada a associação da recidiva vs análise histopatológica das margens cirúrgicas, frequência da ocorrência e dimensão do tumor. Os resultados não evidenciaram associação entre as variáveis estudadas e a mortalidade ou a recidiva. Foi evidenciada a associação entre a ocorrência de recidiva e o tempo de sobrevida no total de casos (p=0,046), encontrando-se esta variável associada a um tempo de sobrevida mediano mais baixo, embora por si só não seja estatisticamente relevante. Não foi evidenciada associação entre o tempo de sobrevida dos casos de fibrossarcoma no local de injecção e as variáveis estudadas. Os resultados obtidos devem ser analisados com precaução, tendo em conta a dimensão reduzida da amostra e a ausência de alguns dados relevantes para a análise.

ABSTRACT - Feline Fibrosarcoma – retrospective study - The class of the skin and soft tissues tumours covers a wide range of neoplasms, among which are the most common in veterinary medicine. The mesenchymal tumours are found in this class, and include, among others, the fibrosarcoma. This neoplasm is considered the most frequent of the skin and subcutaneous tissue and it is common in the cat. There are multiple aetiologies, such as infection by feline leukemia and feline sarcoma viruses, trauma, administration of multiple injections, implantation of microchip and retention of surgical material. The clinical presentation of fibrosarcoma is variable, depending on species, age and aetiology. This tumor may appear as a multiple dimension, soft to firm mass, and does not have a true capsule. It is locally invasive but the capacity of metastasis is low, and it is associated to a high frequency of relapses. Surgical treatment is challenging, and complementation with other therapies such as radiotherapy, chemotherapy and immunotherapy, was already studied. This retrospective study presents the characterization of a sample composed by 34 feline fibrosarcoma cases that were followed at Hospital Veterinário do Restelo, as well as the statistical analysis of variables associated with mortality, survival time and relapses. In this study there was no predisposition of sex or breed. Average age was 9 (±3) years. The infection by feline leukemia and feline sarcoma viruses was not confirmed in any case. The suspected aetiology of injection site fibrosarcomas was vaccination. Only 2 cats had metastasis, but relapses occured in 65% of the cases. The variables associated with mortality and survival time in all cases were age at first diagnosis, sex, tumour location, tumor size, relapse occurrence and the number of surgeries. These events were also studied in the cases of injection-site fibrosarcoma, given its clinical importance. It was investigated the association between relapse occurrence vs surgical margins histopathological analysis, occurrence frequency and tumour size. The results showed no association between the studied variables and mortality or relapses. It was found an association between relapses occurrence and the survival time in all cases (p=0,046), in which this variable was associated with a lower median survival time, although by itself it is not statistically significant. It was not found any association between survival time of injection-site fibrosarcomas and the studied variables. The obtained results should be considered with caution, given the small size of the sample and the absence of some relevant data for the analysis.
