Envelhecimento e inequidades na saúde

Abstract

O envelhecimento, visto como um processo de declínio funcional, apresenta grandes afinidades conceptuais com a saúde. O acesso à saúde, um bem de mérito constituinte das liberdades positivas, é determinante para a qualidade de vida e para a participação activa na sociedade, pelos idosos. As desigualdades na saúde, verificadas no processo de envelhecimento, são injustas quando se associam a desvantagens sociais, mediadas pelos determinantes sociais da saúde — as inequidades. Ajustiça como equidade, de John Rawls, e a abordagem das capacidades, de Amartya Sen, enquadram esta abordagem indirecta à equidade na saúde que deve, sempre, procurar conciliar valores como a liberdade, a igualdade e a eficiência. Este trabalho conclui-se propondo um modelo conceptual que permita futuras explorações metodológicas e empíricas na abordagem à dinâmica das inequidades na saúde verificadas no processo de envelhecimento. Contém quatro vertentes: a precocidade da morte e as incapacidades como variáveis endógenas; a desvantagem social abordada através da privação; a introdução de agregação social multinível no modelo e a construção de tipologias dinâmicas do envelhecimento.

Defining aging as a functional decline process reveals its dose affinities to health. Access to health is a high valuable good constitutive of positive liberties and has a great impact on quality of life and social participation of the aged. Aging process health inequalities are unfair, when associated with disadvantage mediated by 'social determinants of health. Rawls' justice as fairness and Sen's capability approach frame this indirect approach to health equity, based on the concept of inequity. It must, always, balance social values as liberty, equality and efficiency. This work proposes a conceptual model allowing future developments on methodological and empirical approaches to the dynamics of health inequities at the aging process. lis four main components are early death and disability as endogenous variables, social disadvantage represented by deprivation, a multilevel structure of model and the construction of a dynamic typology of aging.
