Comunicar a dor

Abstract

Em Portugal, nas Unidades de Cuidados Paliativos, são utilizados métodos de recolha na avaliação da dor, tais como a Escala Numérica (EN) e a Escala Qualitativa. Estas es-calas são instrumentos unidimensionais, que apenas avaliam a intensidade da dor quando a mesma. É uma experiência multidimensional, dificultando assim a comunica-ção da dor entre o paciente e o enfermeiro. Com a área de Design de Interação e com comunicação visual sintetizada, procurámos explorar e desenvolver um instrumento interativo para obter uma avaliação da multidimensionalidade da dor. Na investigação foi aplicada uma metodologia mista, intervencionista e não intervencionista, de base qualitativa. Na primeira fase, foi feita a revisão da literatura para reunir informação já existente acerca do assunto em questão; um estudo de casos e entrevistas exploratórias sobre a experiência diária da avaliação de dor numa unidade de cuidados paliativos. A partir desta informação iniciou-se a investigação ativa. Foi construído um instrumento interativo, como método de recolha de avaliação da dor multidimensional e sensorial e foi explorada a melhor comunicação da qualidade da dor, a partir do conceito de metáfo-ra para uma linguagem visual, tendo como objetivo uma melhor compreensão e eficácia na resposta do utilizador. No final, foram realizados testes com utilizadores, onde não só o funcionamento e fluxo da interface foram avaliados como o seu conteúdo, nomeada-mente a linguagem visual e metafórica. Chegámos a um instrumento interativo que resultou da obtenção mais eficaz e fundamentada de uma avaliação multidimensional da dor, onde a metáfora contribuiu para a melhoria da comunicação, tradução e a proximidade da experiência da dor. Assim, proporcionámos a recolha de uma avaliação mais completa e informativa possível, para que se fizesse um diagnóstico e a escolha de um tratamento mais adequado ao tipo de dor de cada paciente.

ABSTRACT: In Portugal, in Palliative Care Unit, are applied methods of pain’s measurement collec-tion such as: Numerical Scale (NS) and Qualitative Scale. This scales are unidimensional instruments that measures only the pain intensity, when pain is a multidimensional expe-rience, and that’s why, difficults the communication between the patient and the nurse. With Interaction Design area and synthesized visual communication, we looked to explore and develop an interactive instrument that responds to the pain multidimen-sionality. In this investigation, was applied a mixed methodology, interventionist and non-interven-tionist, of qualitative base. The first phase, started with, a literature revi-sion, to collect existent information about the subject; a cases of study and exploratory interviews, about the measurement of the pain diary experiences, in a palliative care unit. From this infor-mation we initiated the active research. Where an interactive ins-trument was build, as multidimensional and sensorial, method of pain’s measurement, where the best communi-cation of the pain quality was explored, from the metaphor concept to a visual language, that enabled the user a better comprehension and effecti-veness response. In the end, tests with the users were conducted, where not only the performance and the interface flow were tested as its content, in particular, the visual and metaphoric language. We reached an interactive ins-trument, that resulted on get-ting a multidimensional measurement more effective and valid, where the contribution of the metaphor offered a better communi-cation and interpretation, and a pain experi-ence proximity. Therefore, we intented to pro-vide a more complete and informative measurement collection, for a medical diagnosis and treatment, that would be more appropriate for the patient’s pain type.
