Abstract

A ovariohisterectomia é a cirurgia abdominal mais realizada em medicina veterinária (S. F. A. P. F. Brun M. V., Beck C. A. C., Mariano M. B., Mello J. R. B., 2000). A ovariohisterectomia e ovariectomia laparoscópicas bem como a ovariohisterectomia laparoscópica assistida já foram descritas em pacientes veterinários. Estudos anteriores demonstraram que há uma redução da dor quando se comparam estes procedimentos com os homónimos em cirurgia aberta (Mayhew P.D., 2007). Neste trabalho foram realizadas 8 ovariohisterectomias laparoscópicas electivas em cadelas no Departamento de Cirurgia da Faculdade de Medicina Veterinária de Cáceres, Espanha. Os animais eram da raça Galgo e apresentavam idade média de 4,0 ± 1,96 anos e uma média de peso de 22,1 ± 1,86 Kg. A cirurgia foi realizada em 20 passos sequenciais, utilizando para hemostase das estruturas o LigaSure®. Três trocateres foram colocados na linha média para permitir a visualização e manipulação uterina e ováricas, assim como manobras de dissecção, hemostase e exérese. Quanto aos resultados obtidos, as oito cirurgias foram concluídas com sucesso, não havendo necessidade de conversão para técnica aberta em nenhum dos casos. O LigaSure® foi de fácil utilização e apresentou resultados consistentes de excelente hemostase. A cirurgia mais curta demorou apenas 42 minutos. Apenas uma cadela desenvolveu enfisema subcutâneo. As cirurgias realizadas confirmam a viabilidade da técnica para a realização de ovariohisterectomia laparoscópica em cadelas. Tal como noutros estudos, conclui-se que a OVH laparoscópica é um método relativamente rápido, fiável, com claras vantagens no acesso cirúrgico e recuperação rápida e menos dolorosa dos animais no período pós-operatório.

ABSTRACT - Laparoscopic ovariohysterectomy in the female dog - Ovariohysterectomy is the most frequently performed abdominal surgery in veterinary practice (S. F. A. P. F. Brun M. V., Beck C. A. C., Mariano M. B., Mello J. R. B., 2000). Laparoscopic ovariohysterectomy, laparoscopic assisted ovariohysterectomy and laparoscopic ovariectomy have been described in veterinary patients and shown to reduce pain compared with “open” ovariohysterectomy techniques in several studies (Mayhew P.D., 2007). In the present study elective laparoscopic ovariohysterectomies were performed in 8 female dogs in the Small Animal Surgery Department, in the Veterinary University of the Extremadura University in Spain. The animals were from the Galg breed, their mean age was 4,0 ± 1,96 years and they weighted 22,1 ± 1,86 Kg. The procedure was performed in 20 steps and LigaSure® was used for haemostasis of vascular structures. Three trocars were positioned in the midline allowing ovarian and uterine visualization and manipulation, as well as dissection, haemostasis and excision manoeuvres. Concerning the results obtained, all of the surgeries were successful and it was not necessary to convert to the open technique in none of the cases. The use of LigaSure® was easy and it provided an excellent haemostasis. The shortest surgery took only 42 minutes. Only one dog developed subcutaneous emphysema. In this study laparoscopic ovariohysterectomy in the female dog has proven to be a valuable alternative to the open technique. As others have concluded, this surgical procedure is a fast, reliable method, which allows good organ access and visualization and a quick and less painful recovery of the patients in the post-operative period.
