Biocompatibilidade do inserto ineye® em cães

Abstract

A administração tópica de fármacos é a via mais usada para o tratamento de doenças oculares. Porém, esta via requer uma frequência elevada de administrações ao longo do dia, devido à baixa biodisponibilidade do fármaco no local alvo – o bulbo ocular, tornando-se por isso pouco prático e dificultando a adesão à terapêutica pelos detentores dos animais. Por isso, são necessárias novas formas farmacêuticas para administração tópica que possibilitem uma absorção continua dos fármacos. Com este propósito, foi desenvolvido, por uma equipa de investigadores da Universidade de Coimbra, um inserto ocular designado de ineye®. Este inserto, de base polimérica, tem demonstrado elevada biocompatibilidade in vitro, isto em cultura de células de epitélio da córnea. Este estudo propôs-se a avaliar a biocompatibilidade nas vertentes da biofuncionalidade e da tolerância do inserto ocular ineye® in vivo. Para tal, foram selecionados 27 cães mesocefálicos, todos com exames oftalmológicos normais. Os insertos oculares testados, com um tamanho máximo de 4 mm e um peso máximo de 40 mg, podiam ter formato esférico (n=14), hemisférico (n=3), oblongo (n=3) ou formato feijão (n=7). Antes da sua aplicação, foram esterilizados e, em alguns casos, o inserto foi hidratado ou revestido por ácido hialurónico. Os insertos foram colocados no saco conjuntival superior (n=6) ou inferior (n=21) e foram programados exames oftálmicos para as 6 horas, 24 horas e diariamente, até 7 dias pós colocação, para detetar sinais de desconforto ou inflamação ocular (hiperémia conjuntival, congestão, quemose, lesões da córnea, secreções oculares ou prurido). Os resultados demonstraram que em 11,1% (3/27) dos casos, os insertos foram expulsos em poucos minutos após a colocação, em 59,3% (16/27) dos casos num período de tempo inferior a 6 horas e em 25,9 % (7/27) dos casos o inserto permaneceu mais de 6 horas, mas menos de 24 horas e num caso (3,6%) permaneceu mais de 48, mas menos de 72 horas. Em nenhum dos casos foram observados sinais de inflamação ou desconforto. Pode então concluir-se que nas condições experimentais utilizadas, a biofuncionalidade do ineye® foi baixa, o que se poderá ser explicado pela presença da membrana nictitante nos cães e pelas diferenças morfológicas entre as raças de cães incluídas neste estudo. No entanto e devido ás diferenças anatómicas existentes, este resultado não invalida que o inserto possa ser biofuncional na espécie humana. Não se observaram quaisquer efeitos adversos decorrentes da presença do inserto, o que demonstra uma boa tolerância da espécie canina ao ineye®.

ABSTRACT - BIOCOMPATIBILITY OF THE INEYE® INSERT IN DOGS - Topical administrations are the preferred administration route for the treatment of ocular diseases. Howerver, this route requires applications several times a day, due to the drug´s low bioavailability at the target site of the eye, which sometimes becomes unpractical. Therefore, new forms of topical ocular administration of drugs are needed to enable continuous therapeutic delivery. For this purpose, an eye insert designated by ineye® is being developed by a research team at Coimbra University. This polymer-based insert has been previously tested in vitro in cell cultures of corneal epithelium showing high biocompatibility. This study aimed to evaluate the biocompatibility and tolerance of the ineye® insert in vivo, through the evaluation of its biofunctionality and tolerance. 27 mesocephalic dogs with normal ophthalmologic exams were included in the experiment. The tested ocular inserts had a maximum size of 4mm and a maximum weigh of 40 mg, being spherical (n=14), hemispherical (n=3), oblong (n=3) or bean shaped (n=7). In addition, they were sterilized before application and, in some cases, the insert was hydrated or covered with hyaluronic acid. The inserts were placed on the upper (n=6) or lower (n=21) conjuntival sac and ophthalmic examinations were scheduled post 6 hours, post 24 hours and daily until 7 days post placement. Thees examinations aimed to recognize signs of discomfort or inflammation (such as conjunctival hyperemia, congestion, chemosis, corneal lesions, ocular discharges or pruritus),. The results showed that in 11,1% (3/27) of the cases, the inserts were expelled few minutes after their placement. In 59,3% (16/27) of the cases, this happened before 6 hours and in 25,9% (7/27) of the cases they stayed over 6 hours, but less than 24 hours. In only one case (3,6%), the insert remained for more than 48 hours, but less than 72 hours. There were no signs of inflammation or discomfort due to their presence. This study aimed to evaluate biofunctionality and tolerance of the ineye® insert in dogs. Under these experimental conditions, biofunctionality was low, which could be due to the presence of the nictitating membrane and to morphologic differences between breeds. Due to the anatomical differences, this does not mean that the insert can´t be biofunctional in humans. There were no clinical signs associated with the presence of the insert, presenting a good tolerance in dogs.
