Idade à ovariohisterectomia e o desenvolvimento de neoplasias malignas não reprodutivas em cadelas

Abstract

Em 2019, a ovariohisterectomia e a orquiectomia eram as cirurgias mais frequentes em cães, com vantagens no controlo populacional, na gestão de diversos comportamentos caninos, na prevenção de doenças do aparelho reprodutor e de neoplasias mamárias. No entanto, a gonadectomia tem vindo a ser associada a um vasto conjunto de problemas. São exemplos disso os riscos da cirurgia em si e de possíveis complicações pós-cirúrgicas, a obesidade, bem como afeções do trato urinário, endócrinas, ortopédicas e imunitárias. Além disso, tem vindo a ser associada ao aparecimento de determinadas neoplasias, tais como tumores prostáticos, mastocitoma, hemangiossarcoma, linfoma, osteossarcoma e carcinoma do urotélio. As alterações endócrinas precipitadas pela remoção das gónadas poderão influenciar a dimensão dos problemas associados, dependendo da idade na qual é realizada a cirurgia. Assim, este trabalho teve como objetivo principal verificar se a ovariohisterectomia (OVH) e a idade na qual é feita poderá estar associada, de forma transversal, ao desenvolvimento de neoplasias malignas não reprodutivas (dos órgãos reprodutivos e mamárias), em cadelas. Este estudo retrospetivo de caso-controlo emparelhado, incluiu 64 casos e 64 controlos, com dados obtidos de relatórios de análise patológica e das plataformas de registo do Hospital Escolar Veterinário – Faculdade de Medicina Veterinária – Universidade de Lisboa. Foi calculado o Odds Ratio usando a regressão linear condicional e o teste de McNemar. Numa amostra com 21 tipos histológicos de neoplasia e com 20 raças diferentes (incluindo sem raça definida), as cadelas esterilizadas apresentaram um risco 3 vezes superior (P<0,01) de desenvolver as neoplasias em estudo. Quando a OVH foi feita até aos 2 anos de idade, este risco foi 7 vezes superior (P<0,05) em relação às cadelas esterilizadas depois dos 2 anos e inteiras. Verificou-se também um aumento no risco de neoplasias hematopoiéticas e linfoides (mastocitomas e linfomas) com a esterilização até aos 2 anos de idade. Este estudo demonstra um impacto transversal da OVH, principalmente quando ocorre até aos 2 anos de idade, sobre as neoplasias investigadas, que poderá ir além das neoplasias com maior foco na literatura

ABSTRACT - In 2019, ovariohysterectomy and orchiectomy were the most frequent surgeries in dogs, with advantages in population control, managing various canine behaviors, and preventing reproductive system diseases and mammary cancer. However, gonadectomy has been associated with a wide range of problems. Examples include the risks related to the surgery itself, potential post-surgical complications, obesity, as well as urinary tract, endocrine, orthopedic, and immune disorders. Additionally, it has been linked to the emergence of specific neoplasms, such as prostatic tumors, mast cell tumors, hemangiosarcoma, lymphoma, osteosarcoma, and transitional cell carcinoma. Endocrine changes triggered by gonad removal may influence the extent of these problems, depending on the age at which the surgery is performed. Thus, the primary objective of this study was to verify whether ovariohysterectomy (OVH) and the age at which it is performed are broadly associated with the development of malignant neoplasms in female dogs, excluding reproductive system neoplasms (which include mammary neoplasms). This matched retrospective case-control study included 64 cases and 64 controls, with data obtained from pathological analysis reports and the registry platforms of the Veterinary Teaching Hospital – Faculty of Veterinary Medicine – Lisbon University. Odds Ratios were calculated using conditional linear regression and McNemar's test. In a sample including 21 histological types of neoplasms and 20 different breeds (including mixed breeds), sterilized female dogs showed a threefold higher risk (P<0.01) of developing the neoplasms under study. When OVH was performed before two years of age, this risk was seven times higher (P<0.05) compared to females undergoing OVH after two years or remaining intact. An increased risk of hematopoietic and lymphoid neoplasms (mast cell tumors and lymphomas) was also observed with sterilization before two years of age. This study demonstrates a broad impact of OVH, particularly when performed before two years of age, on the neoplasms under investigation, potentially extending beyond those most commonly discussed in the literature
