Interações entre suplementos alimentares e medicamentos

Abstract

Apesar da ciência ter evoluído e os tratamentos disponíveis no mercado serem cada vez mais inovados e eficazes, o consumo de suplementos alimentares tem aumentado nos últimos 20 anos em todo o mundo, impulsionado pelo marketing e influência da indústria, pela crença de que “o natural é melhor e mais seguro” e pelo fácil acesso aos mesmos, uma vez que não é necessário prescrição médica. Os suplementos alimentares definem-se como géneros alimentícios que se destinam a complementar e/ou suplementar o regime alimentar normal, constituindo fontes concentradas de determinadas substâncias, como vitaminas ou nutrientes. As consequências deste aumento tornaram-se um desafio para os sistemas de saúde, uma vez que a sua ingestão simultânea com medicamentos pode ter forte um impacto na libertação, absorção, distribuição, metabolismo e/ou eliminação dos fármacos. Embora algumas interações possam ser benéficas para os doentes como, por exemplo, as vitaminas e o cálcio que ajudam nos efeitos secundários da quimioterapia, a grande maioria é prejudicial e pode resultar em danos sérios para a saúde, como é o caso da suplementação de vitamina K em doentes que fazem terapia anticoagulante, uma vez que pode aumentar o risco de formação de coágulos, resultando em eventos tromboembólicos graves, como o acidente vascular cerebral ou trombose venosa profunda. O facto de, muitas vezes, os doentes não reportarem aos profissionais de saúde toda a medicação e suplementação que tomam agrava este problema. Os farmacêuticos, no entanto, podem desempenhar um papel crucial na mitigação desses riscos, pois geralmente têm acesso ao histórico de medicação crónica do doente e, muitas vezes, são o ponto de compra da suplementação, tentando, deste modo, obter o máximo de benefícios e o mínimo de interações medicamentosas. Esta monografia fornece informações sobre diversas interações entre diferentes suplementos alimentares e medicamentos, explicando os mecanismos envolvidos, com o intuito de contribuir para a prevenção de possíveis interações entre suplementos alimentares e medicamentos.

Although science has evolved and the treatments available on the market are increasingly innovative and effective, the consumption of dietary supplements has increased in the last 20 years throughout the world, driven by marketing and industry influence, by the belief that “the natural it’s better and safer” and because of the easy access to them, as there is no need for a medical prescription. Food supplements are defined as foodstuffs that are intended to complement and/or supplement the normal diet, constituting concentrated sources of certain substances, such as vitamins or nutrients. The consequences of this increase have become a challenge for healthcare systems, since their simultaneous intake with medications can have a strong impact on the release, absorption, distribution, metabolism and/or elimination of drugs. Although some interactions may be beneficial for patients, such as vitamins and calcium that help with the side effects of chemotherapy, the vast majority are harmful and can result in serious harm to health, as is the case with vitamin K supplementation in patients undergoing therapy anticoagulant, as it can increase the risk of clot formation, resulting in serious thromboembolic events, such as stroke or deep vein thrombosis. The fact that patients often do not report all the medication and supplementation they take to healthcare professionals exacerbates this problem. Pharmacists, however, can play a crucial role in mitigating these risks, as they often have access to a patient's chronic medication history and are often the point of purchase for supplementation, thereby trying to obtain maximum benefits. and minimal drug interactions. This monograph provides information on various interactions between different dietary supplements and medications, explaining the mechanisms involved, with the intention of contributing to the prevention of possible interactions between dietary supplements and medications.
