Abstract

O presente relatório descreve e analisa reflexivamente as atividades desenvolvidas no âmbito do estágio de natureza profissional integrado no Mestrado em Enfermagem Médico-cirúrgica, na área de Enfermagem à Pessoa em Situação Perioperatória. O objetivo central consistiu no desenvolvimento de competências clínicas especializadas, com foco na manutenção da normotermia no cliente submetido a cirurgia de retalho livre. Adotou-se uma abordagem descritiva e crítico-reflexiva, sustentada pela evidência científica, incluindo a análise de um estudo de caso e a realização de uma scoping review. O estágio decorreu no bloco operatório central e na unidade de cirurgia de ambulatório de uma Unidade Local de Saúde, sob um modelo de tutoria contínua. Ao longo do estágio, foram explorados os desafios éticos e legais inerentes à prática perioperatória, nomeadamente a autonomia e a vulnerabilidade do cliente, com ênfase na garantia do consentimento informado e na tomada de decisão ética. A segurança do cliente emergiu como um pilar fundamental, abordando a prevenção de complicações como a hipotermia e a infeção do local cirúrgico. No âmbito da melhoria contínua da qualidade, foi desenvolvida e proposta a implementação de um protocolo de gestão da ansiedade préoperatória na Unidade de Cirurgia de Ambulatório, visando uma abordagem personalizada às necessidades dos clientes. Uma scoping review sobre a manutenção da normotermia em cirurgia de retalho livre permitiu mapear as melhores práticas baseadas na evidência científica. O estágio contribuiu significativamente para o desenvolvimento da praxis clínica especializada, alicerçada na evidência científica e na reflexão sobre a prática. Os cuidados planeados com apoio da plataforma e4nursing baseada na Ontologia de Enfermagem e a reflexão sobre o modelo Perioperative Patient Focused Model da AORN evidenciam o compromisso com a conceção cuidados centrados no cliente. A experiência de estágio proporcionou um crescimento profissional e pessoal relevante, consolidando competências essenciais para a prestação de cuidados de enfermagem de excelência na área perioperatória.

This report describes and reflectively analyzes the activities developed within the scope of the professional internship integrated into the Master's Degree in Medical-Surgical Nursing, in the area of Nursing for the Person in Perioperative Situation. The central objective consisted of the development of specialized clinical skills, with a focus on maintaining normothermia in clients undergoing free flap surgery. A descriptive and critical-reflective approach was adopted, supported by scientific evidence, including the analysis of a case study and the performance of a scoping review. The internship took place in the central operating theater and the ambulatory surgery unit of a Local Health Unit, under a continuous mentorship model. Throughout the internship, the ethical and legal challenges inherent to perioperative practice, namely client autonomy and vulnerability, were explored, with an emphasis on ensuring informed consent and ethical decision-making. Patient safety emerged as a fundamental pillar, addressing the prevention of complications such as hypothermia and surgical site infections. In the context of continuous quality improvement, the development and proposed implementation of a preoperative anxiety management protocol in the Ambulatory Surgery Unit was carried out, aiming for a personalized approach to client needs. The scoping review on the maintenance of normothermia in free flap surgery allowed for the mapping of best practices based on scientific evidence. The internship significantly contributed to the development of specialized clinical praxis, grounded in scientific evidence and reflection on practice. The planned care supported by the e4nursing platform, based on Nursing Ontology, and the reflection on the AORN's Perioperative Patient Focused Model demonstrate a commitment to client-centered care design. The internship experience provided relevant professional and personal growth, consolidating essential competencies for delivering excellent nursing care in the perioperative area.
