Abstract

A língua é um órgão essencial da cavidade oral, desempenhando funções fundamentais como sucção, deglutição, mastigação e articulação da fala. A sua mobilidade adequada é crucial para o desenvolvimento do sistema estomatognático e para o crescimento harmonioso do recém-nascido. Alterações anatómicas e funcionais do freio lingual, como a anquiloglossia, podem limitar a movimentação da língua, interferindo na amamentação e comprometendo o ganho de peso, a coordenação entre respiração, sucção e deglutição, bem como o desenvolvimento fonético da criança. A frenotomia e a frenectomia lingual são intervenções cirúrgicas frequentemente recomendadas para bebés com anquiloglossia sintomática. A frenotomia é normalmente realizada até ao primeiro ano de vida, devido à sua simplicidade e rápida execução. Em crianças mais velhas, sobretudo a partir dos dois anos, recomenda-se preferencialmente a frenectomia, que também pode ser realizada a partir do primeiro ano quando existem limitações que afetam a fala ou outras funções orais. Ambos os procedimentos são minimamente invasivos e apresentam um prognóstico favorável quando indicados de forma adequada e fundamentada. A avaliação detalhada das características anatómicas e funcionais do freio lingual constitui um instrumento essencial para a decisão clínica, permitindo identificar situações em que a intervenção cirúrgica é necessária e evitando procedimentos desnecessários. Esta revisão narrativa tem como objetivo analisar a relação entre a anatomia e função do freio lingual e os padrões de sucção e deglutição durante a amamentação, sublinhando a importância do conhecimento especializado para fundamentar intervenções cirúrgicas. Destaca-se, ainda, a relevância do diagnóstico precoce e da abordagem interdisciplinar na gestão da anquiloglossia, promovendo práticas seguras, eficazes e centradas no bem-estar e desenvolvimento global da relação mãe-bebé.

The tongue is an essential organ of the oral cavity, performing fundamental functions such as sucking, swallowing, chewing, and speech articulation. Adequate tongue mobility is crucial for the development of the stomatognathic system and for the harmonious growth of the newborn. Anatomical and functional alterations of the lingual frenulum, such as ankyloglossia, can restrict tongue movement, interfering with breastfeeding and affecting weight gain, the coordination of breathing, sucking, and swallowing, as well as the child’s phonetic development. Lingual frenotomy and frenectomy are surgical interventions frequently recommended for infants with symptomatic ankyloglossia. Frenotomy is typically performed within the first year of life due to its simplicity and rapid execution. In older children, especially from two years of age, frenectomy is generally preferred, though it can also be performed from the first year when limitations affect speech or other oral functions. Both procedures are minimally invasive and have a favorable prognosis when appropriately and thoroughly indicated. A detailed evaluation of the anatomical and functional characteristics of the lingual frenulum is essential for clinical decision-making, enabling the identification of cases that truly require surgical intervention while avoiding unnecessary procedures. This narrative review aims to analyze the relationship between the anatomy and function of the lingual frenulum and the patterns of sucking and swallowing during breastfeeding, emphasizing the importance of specialized knowledge to guide surgical interventions. Additionally, it highlights the relevance of early diagnosis and an interdisciplinary approach in the management of ankyloglossia, promoting safe, effective practices focused on the well-being and overall development of the mother–infant dyad.
