Bisfosfonato e implantologia

Abstract

A implantologia é um tratamento recomendado em pacientes edêntulos. O seu sucesso baseia-se na estabilidade do implante associada a uma boa osteointegração, que pode ser alterada por vários fatores, nomeadamente a toma de bisfosfonatos. Os bisfosfonatos são fármacos, que diminuem a atividade osteoclástica que têm sido utilizados favoravelmente na osteointegração uma vez que inibem a reabsorção óssea. Os bisfosfonatos são utilizados no tratamento de doenças ósseas como a doença de Paget ou osteoporose (1). Os efeitos biológicos fundamentais dos bifosfonatos são a inibição da calcificação e da reabsorção óssea nestas doenças. Mas apresentam efeitos adversos como a osteonecrose dos maxilares, cujo mecanismo induzido pelos bisfosfonatos é ainda desconhecido. No entanto, existem fatores de risco já identificados. As terapêuticas de implantes dentários e os tratamentos com bisfosfonatos não são incompatíveis (2). Vários estudos demonstram que em doentes tratados com bisfosfonatos orais a colocação de implantes não aumenta a incidência de osteonecrose, dependendo da duração do tratamento. No entanto, há um aumento significativo da incidência de osteonecrose em doentes tratados com bisfosfonatos intravenosos (3).
