Osteonecrose maxilar associada ao uso de bifosfonatos

Abstract

Os bifosfonatos (BFs) são medicamentos usados principalmente no tratamento de algumas complicações de cancros sólidos, osteoporose, mieloma múltiplo e doença de Paget. Desde 2002 começaram a surgir pela Food and Drug Administration (FDA), relatórios de osteonecrose nos maxilares em doentes medicados com estes fármacos, inicialmente em doentes em tratamento de cancro, e posteriormente pela sua utilização na osteoporose. A etiopatogenia da osteonecrose dos maxilares não está completamente compreendida. No entanto, sabe-se que entre uma variedade de possíveis fatores etiológicos, a inibição do recrutamento e a apoptose dos osteoclastos (fundamentais na redução da reabsorção óssea) causadas por estes fármacos, tornam-se um obstáculo quando existe necessidade de uma normal regeneração alveolar, por estas células serem elementos fundamentais nas primeiras fases desta. Assim, a osteonecrose dos maxilares associada ao tratamento com bifosfonatos constitui uma complicação oral grave, que pode ser iniciada nestes doentes após um tratamento dentário comum e progredir para a exposição crónica de osso necrosado com as possíveis evoluções, complicações e consequências. O objetivo deste trabalho é rever as indicações, mecanismos de ação, farmacocinética e efeitos adversos dos bifosfonatos e clarificar a fisiopatologia, sinais e sintomas, fatores de risco, estadiamento e protocolo terapêutico deste tipo de osteonecrose.
