Abstract

Os incidentalomas adrenais são massas adrenais detetadas ocasionalmente através de técnicas de diagnóstico por imagem. Constituem achados cada vez mais frequentes na prática clínica médico-veterinária, uma vez que a realização de exames de diagnóstico por imagem como a ecografia, tomografia computorizada (TC) e ressonância magnética (RM) são mais frequentes. Depois de detetada a massa adrenal, deve ser determinada a sua funcionalidade – massa funcional ou não funcional – de acordo com a capacidade de produção hormonal, e, nos casos funcionais, devem ser determinadas as hormonas que produz. Deve ser também determinada a natureza da massa – não tumoral, tumoral benigna, maligna ou metastática. As características da massa adrenal identificadas nos exames imagiológicos podem sugerir o caráter benigno ou maligno da lesão. A abordagem terapêutica e o prognóstico são variáveis de acordo com a natureza da massa adrenal. Nos casos em que as massas adrenais são não funcionais e manifestam critérios de benignidade nos exames imagiológicos, devem ter um acompanhamento clínico periódico. Nos casos em que as massas adrenais manifestam critérios de malignidade nos exames imagiológicos, a adrenalectomia constitui o tratamento de eleição na maioria das situações.
