Abstract

A asma é um conjunto de condições heterogéneas que resulta na obstrução reversível dos brônquios, e é caracterizada por hiper-reatividade e inflamação das vias aéreas. Existem vários fatores que podem levar ao aparecimento da doença, como predisposições genéticas, interações com o ambiente e fatores socioeconómicos. A compreensão dos mecanismos imunológicos e patogénicos da doença foi bastante aprofundada na última década e sabe-se agora que a inflamação que está na base da doença é o resultado do envolvimento uma grande variedade de células e mediadores inflamatórios. Os corticosteroides inalados formam a primeira linha do controlo da asma e têm demonstrado a sua eficácia e segurança. Os casos mais graves de asma continuam, no entanto, a ser um desafio em que, muitas vezes, não se consegue atingir o controlo desejado mesmo utilizando as combinações de fármacos recomendadas. Novas terapias biológicas estão a surgir para contrariar esta problemática, como é o caso do anticorpo monoclonal Anti-IgE, Omalizumab, ou os anticorpos monoclonais Anti-IL5, Anti-IL4 e Anti-IL13, ainda em ensaios clínicos.
