Comparação do desempenho do Sysmex UF-4000 com urocultura para o diagnóstico de infeções do trato urinário

Abstract

O presente relatório descreve as atividades realizadas no estágio curricular, e foi dividido em dois capítulos. O primeiro corresponde à descrição dos setores laboratoriais e equipamentos utilizados na determinação dos parâmetros analíticos, e às competências desenvolvidas. O segundo capítulo corresponde a um estudo caso denominado “Comparação do desempenho do Sysmex UF-4000 com Urocultura para o diagnóstico de infeções do Trato Urinário”. O objetivo foi comparar os resultados obtidos pelo equipamento SysmexUF-4000 e Urocultura, relativamente ao tipo de bactéria. Foram analisadas 545 urinas com suspeita de ITU. As amostras foram consideradas positivas pelo Sysmex se valores de densidade bacteriana fossem ≥200µ e/ou leucócitos ≥45µ e em cultura, caso o crescimento bacteriano fosse ≥105 UFC/ml. Das 545 amostras analisadas, 108 são discordantes. Obteve-se uma sensibilidade = 94%, especificade= 87%, VPN=97%, VPP=73% e um coeficiente de Kappa de Cohen= 0,786. A boa concordância, permite-nos afirmar que a implementação do método automatizado na rotina laboratorial é uma mais-valia na rapidez dos resultados e na diminuição dos exames culturais de urina. No entanto, deverão ser realizados mais estudos para aferir um valor d cut-off de acordo com as variáveis: sexo, idade, patologias e os níveis de contagem de colónias.
