Abstract

Introdução: O microbioma humano, composto por bactérias, fungos, vírus e seu material genético, desempenha um papel essencial na saúde e nas doenças humanas. O microbioma oral, em particular, tem sido objeto de estudos relacionados com o cancro, com evidências sugerindo associações entre a disbiose oral e a carcinogénese. O cancro é caracterizado pelo crescimento descontrolado de células aberrantes, sendo um problema significativo de saúde pública global. É por isso necessário identificar modificações específicas associadas ao cancro, que possam ser usadas como biomarcadores eficazes para o diagnóstico ou prognóstico desta doença. O presente trabalho teve como objetivo identificar alterações no microbioma oral de pacientes com cancro, que possam ter relevância para o seu diagnóstico ou prognóstico, através de uma revisão sistemática da literatura. Materiais e métodos: A revisão foi conduzida seguindo as guidelines PRISMA. A pesquisa da literatura foi realizada em 2024 e inclui artigos dos últimos 10 anos, nas bases de dados bibliográficas PubMed/MEDLINE®, Web of Science/MEDLINE® e Scopus® usando o seguinte esquema de pesquisa: “(mouth [MeSH] OR saliva [MeSH] OR oral) AND (cancer OR neoplasms [MeSH]) AND (microbiome OR microbiota [MeSH]) AND ("changes" OR "variations" OR "alterations")”. Resultados: Os estudos selecionados foram avaliados quanto à sua qualidade metodológica e características descritivas. O principal tipo de cancro estudado foi o cancro oral e o principal tipo de amostra usada nos estudos foi saliva. Os resultados mostraram que certos géneros bacterianos estão aumentados em pacientes com cancro, como é o caso de Fusobacterium e Prevotella, enquanto outros estão diminuídos, por exemplo, Streptococcus e Rothia. Conclusão: O microbioma oral desempenha um papel significativo na saúde e em doenças, incluindo o cancro. A disbiose oral está associada a processos tumorais, influenciando a inflamação crónica e a resposta imune. Compreender estas interações pode levar ao desenvolvimento de novas estratégias terapêuticas e melhorias nos desfechos de tratamento para pacientes com cancro. São necessários mais estudos para elucidar as complexas relações entre o microbioma oral e o cancro.

Introduction: The human microbiome, made up of bacteria, fungi, viruses and their genetic material, plays an essential role in human health and disease. The oral microbiome, in particular, has been the subject of cancer-related studies, with evidence suggesting associations between oral dysbiosis and carcinogenesis. Cancer is characterized by the uncontrolled growth of aberrant cells and is a significant global public health problem. It is therefore necessary to identify specific changes associated with cancer, which can be used as effective biomarkers for the diagnosis or prognosis of this disease. The aim of this study was to identify alterations in the oral microbiome of cancer patients that may be relevant to their diagnosis or prognosis, through a systematic review of the literature. Materials and methods: The review was conducted following the PRISMA guidelines. The literature search was carried out in 2024 and includes articles from the last 10 years, in the bibliographic databases PubMed/MEDLINE®, Web of Science/MEDLINE® and Scopus® using the following search scheme: "(mouth [MeSH] OR saliva [MeSH] OR oral) AND (cancer OR neoplasms [MeSH]) AND (microbiome OR microbiota [MeSH]) AND ("changes" OR "variations" OR "alterations")". Results: The selected studies were assessed for their methodological quality and descriptive characteristics. The main type of cancer studied was oral cancer and the main type of sample used in the studies was saliva samples. The results showed that certain bacterial genera are increased in cancer patients, such as Fusobacterium, while others are decreased, such as Streptococcus. Conclusion: The oral microbiome plays a significant role in health and diseases, including cancer. Oral dysbiosis is associated with tumor processes, influencing chronic inflammation and the immune response. Understanding these interactions could lead to the development of new therapeutic strategies and improvements in treatment outcomes for cancer patients. More studies are needed to elucidate the complex relationships between the oral microbiome and cancer.
