O movimento da alta social em contexto hospitalar

Abstract

A relação da Saúde com o Serviço Social é parte integrante da história da profissão e tradicionalmente um dos campos de intervenção da profissão. O conceito de saúde temse vindo a modificar com o tempo e nos dias de hoje é um conceito mais vasto que engloba a melhoria da qualidade de vida associado à promoção da saúde, sendo um direito fundamental a todo o ser humano. O “Assistente Social é um profissional da intervenção social com uma prática inter e transdisciplinar, que atua com e para as pessoas, numa lógica de cooperação.” (Associação dos Profissionais de Serviço Social, 2018:6). No contexto de saúde, o assistente social assume essencialmente uma função de mediação entre o doente a família e os diferentes serviços, de forma a ir ao encontro da garantia do seu bem-estar e qualidade de vida, através do acompanhamento do internamento. A alta em contexto hospitalar requer trabalho de equipa e articulação, para que seja assegurada a garantia da continuidade dos cuidados de saúde. O planeamento da alta ocorre para junto da família ou para uma resposta da comunidade (Cf. Masfret), sendo que os dois podem ser mobilizados conforme necessidade. O processo da alta social inicia-se com a fase de acolhimento, aprofundamento da situação, delineação de um plano através do acompanhamento e, por fim a avaliação da mesma.

The relationship between Health and Social Work is an integral part of the profession's history and traditionally one of the profession's fields of intervention. The concept of health has been changing over time and nowadays it is a broader concept that encompasses the improvement of quality of life associated with health promotion, being a fundamental right of every human being. The “Social Worker is a social intervention professional with an inter and transdisciplinary practice, who works with and for people, in a cooperation logic.” (Associação dos Profissionais de Serviço Social, 2018:6). In the context of health, the social worker essentially assumes a role of mediation between the patient, the family, and the different services, in order to guarantee their well-being and quality of life, through monitoring the hospitalization. Discharge in a hospital context requires teamwork and articulation, so that the continuity of health care is ensured. Discharge planning takes place with the family or for a community response (Cf. Masfret), both of which can be mobilized as needed. The process of social discharge begins with the reception phase, deepening the situation, delineating a plan through monitoring and, finally, its evaluation.
