Caraterização da saúde oral e microbioma oral em pacientes com patologia gastrintestinal do Centro Hospitalar Tondela-Viseu

Abstract

Introdução: A saúde e microbioma oral encontram-se associadas com a doença inflamatória intestinal (DII). Assim, surge a necessidade de uma caracterização exaustiva dos pacientes portadores desta doença com vista a uma adequada abordagem terapêutica. Os objetivos deste estudo incluíram: identificar os aspetos sócio-demográficos, comportamentos de saúde oral e hábitos diários em pacientes com doença inflamatória intestinal; caracterizar a saúde oral dos pacientes com doença inflamatória intestinal; analisar o microbioma oral de indivíduos que recorrem à Clínica Dentária Universitária (CDU) e potencial associação com características demográficas e de saúde oral e sistémica, e relacionar os perfis de microbioma oral com o estado de saúde oral e sistémica. Métodos: O presente estudo observacional transversal assentou em 3 estratégias de recolha de dados. As características sociodemográficas, comportamentos de saúde oral e hábitos diários bem como os aspetos de saúde sistémica foram avaliados através de dois questionários distintos, respetivamente. Foi igualmente efetuado um exame intraoral com vista à condição dentária geral e periodontal e, por último, realizou-se a recolha de material biológico de saliva e biofilme a uma amostra final de 14 pacientes diagnosticados com DII, 50% com Doença de Crohn e 50% com Colite Ulcerativa, reencaminhados do serviço de Gastrenterologia do Centro Hospitalar Tondela-Viseu para consulta na CDU da UCP. Resultados: O grau de escolaridade mais representado foi o Médio (35,71%), entre o 9º e o 12º anos de escolaridade; 57,14% dos participantes apresenta alguma forma de periodontite ou gengivite, onde 21,42% são portadores do estadio mais avançado de periodontite. O índice CPOD é de 7,07 e metade dos participantes apresenta um CPOD superior a 5. Verificou-se uma carga total microbiana menor nos pacientes com CU do que com DC. Caraterização da saúde oral e microbioma oral em pacientes com patologia gastrintestinal do Centro Hospitalar Tondela-Viseu Ano letivo 2021/2022 Conclusões: A realização deste estudo permitiu alargar o conhecimento dos fatores como a saúde e o microbioma oral associados à DII. A caracterização detalhada dos pacientes associado à quantificação da carga total microbiana em pacientes com esta doença permite abrir portas a futuras investigações com vista a adequar protocolos terapêuticos a cada paciente.

Introduction: Oral health and microbiome are associated with inflammatory bowel disease (IBD). Thus, there is a need for an ostensive characterization of patients with this disease with a view to an adequate therapeutic approach. The objectives of this study included: knowing the socio-demographic aspects, oral health behaviors and daily habits in patients with inflammatory bowel disease; characterize the oral health of patients with inflammatory bowel disease; analyze the oral microbiome of individuals who use the University Dental Clinic (CDU) and potential association with demographic and oral and systemic health characteristics and to relate oral microbiome profiles with the oral and systemic health status. Methods: The present observational cross-sectional study was based on 3 data collection strategies. Sociodemographic characteristics, oral health behaviors and daily habits as well as systemic health aspects were evaluated using two different questionnaires, respectively. An intraoral examination was also carried out with a view to the general dental and periodontal condition and, finally, biological material from saliva and biofilm was collected from the final sample of 14 patients diagnosed with IBD, 50% with Crohn’s Disease and 50% with Ulcerative Colitis, referred from the Gastroenterology service of Centro Hospitalar Tondela-Viseu for dental appointments at the CDU of the UCP. Results: The most represented level of schooling was Middle (35.71%), between the 9th and 12th grades; 57.14% of the participants have some form of periodontitis or gingivitis, where 21.42% have the most advanced stage of periodontitis. The DMFT index is 7.07 and half of the participants have a DMFT greater than 5. There was a lower total microbial load in patients with Ulcerative Colitis than with Crohn’s Disease. Caraterização da saúde oral e microbioma oral em pacientes com patologia gastrintestinal do Centro Hospitalar Tondela-Viseu Conclusions: This study allowed us to broaden our knowledge of factors such as health and the oral microbiome associated with IBD. The detailed characterization of patients associated with the quantification of the total microbial load in patients with this disease opens the door to future investigations with a view to adapting therapeutic protocols to each patient.
