Abstract

O Transtorno Explosivo Intermitente (TEI), classificado como um tipo de Transtorno do Controle dos Impulsos (TCIs), se caracteriza por episódios graves e isolados de agressividade de forma desproporcional ao evento que os desencadearam. Os episódios são precedidos, na maioria das vezes, por um fator estressante e, sucedidos por intenso arrependimento. O TEI, apesar de existir na literatura há pouco mais de 30 anos, ainda é uma desordem mental bastante desconhecida. Em relação à sociedade, esta condição é considerada por diversas vezes um ato temperamental de um indivíduo que reage grosseiramente aos fatos, sendo levada em conta apenas a partir do momento em que começa a prejudicar a rotina do mesmo ou de terceiros. Portanto, o que os leva a procurar ajuda são as sérias conseqüências do transtorno. Para psiquiatras e psicólogos, esta desordem pode passar despercebida, especialmente quando está correlacionada com outros transtornos ou mesmo quando os pacientes não consideram as explosões agressivas como doentias. O tratamento farmacológico do comportamento agressivo e da impulsividade associado à terapia cognitivo-comportamental, encontra-se bem definido, porém ainda não há um estudo que defina rigorosamente uma terapia medicamentosa para os portadores do TEI. As classes de medicamentos que tem mostrado boa eficácia são antiepilépticos, estabilizadores do humor, antipsicóticos, betabloqueadores e inibidores seletivos da recaptação de serotonina. Esta pesquisa tem como objetivo fazer uma revisão dos principais aspectos do TEI, a partir da leitura, cotejo e análise da literatura existente sobre o tema, produzida nos últimos 10 anos, até agosto de 2013.

The Intermittent Explosive Disorder (IED), classified as a type of an Impulse Control Disorder (ICD), is characterized by severe and isolated episodes of aggression on a disproportionate way to the event that originates them. The episodes are preceded, in most cases, by a stress factor and, succeeded by intense regret. The IED, although it exists in the literature for over 30 years, is still a properly unknown mental disorder. Considering the society, this condition is considered several times a temperamental act of an individual who reacts violently to the facts, only mattering the moment that starts to harm their own routine or others. So, what leads them to seek help are the serious consequences of the disorder. For psychiatrists and psychologists, this disorder can go unnoticed, especially when correlated with other disorders or even when patients do not consider the aggressive explosions as unhealthy. The pharmacological treatment of aggressive behavior and impulsivity associated with cognitive- behavioral therapy, is well defined, but there isn't any study that defines healthy therapy for patients with IED. The classes of medications that have shown good efficacy are antiepileptics, mood stabilizers, antipsychotics, betablockers and selective inhibitors from reuptake of serotonin. This work aims to review the main aspects of IED, from reading, collecting and analysis of existing literature about the subject produced over the last 10 years, until August 2013.
