Varizes ganglionares: fisiopatologia e aspectos ecográficos

Abstract

INTRODUÇÃO: As veias gânglionares presentes no triângulo femoral constituem uma rede venosa subcutânea que liga vários pontos do sistema venoso superficial e profundo. A dilatação desta rede venosa pode ser causa de insuficiência do sistema venoso superficial ou, mais frequentemente, estar envolvida no processo de recorrência varicosa pós-safenectomia. OBJECTIVOS: Descrever os aspectos anatómicos e fisiológicos da rede venosa ganglionar do triângulo femoral, alertando para uma causa pouco conhecida de varizes. MATERIAL E MÉTODOS: Revisão teórica dos mecanismos de formação e implicações clínicas das varizes ganglionares, a que se associa uma análise pictórica dos casos detectados por ecodoppler no nosso serviço. RESULTADOS: As veias ganglionares tornam-se refluxivas quando fenomenos de neovascularização as conectam a troncos venosos insuficientes. Estes vasos são visiveis ecograficamente, geralmente com múltiplos trajectos trans-ganglionares. A sua detecção tem implicações terapêuticas, constituindo uma contra-indicação relativa à cirurgia, propondo-se actualmente a escleroterapia como melhor método terapêutico. CONCLUSÃO: A pesquisa de uma rede venosa linfo-ganglionar dilatada e insuficiente deve ser feita de forma sistemática em todos os ecodopplers executados no contexto de insuficiência venosa dos membros inferiores, já que a sua identificação contribui para um tratamento dirigido.
