Abstract

Resumo: O prurido é essencialmente um sintoma de origem cutânea, mas a sua etiologia pode dever-se a causas dermatológicas, neuropáticas, sistémicas ou psicogénicas. O tratamento do prurido persistente e intenso é desafiante e exige do clínico o conhecimento dos mecanismos subjacentes. A histamina é o principal mediador no prurido de origem cutânea e, deste modo, assiste-se a uma boa resposta ao tratamento com anti-histamínicos. No entanto, quando o prurido é causado por uma condição sistémica, a compreensão da sua fisiopatologia permitiu a uma abordagem terapêutica mais direccionada, usando antagonistas opióides, antidepressivos, fototerapia ultravioleta B e andrógenos, entre outros fármacos. O prurido intratável merece o mesmo grau de dedicação terapêutica que a dor. É um problema comum e que causa tanto desconforto como a dor crónica. Com base em estudos recentes, este trabalho tenta fazer uma revisão da neurofisiologia subjacente a cada condição que se manifesta com prurido e visa estabelecer uma orientação clínica sobre o diagnóstico e tratamento do prurido. Abstract: Pruritus is essentially a cutaneous symptom but its etiology may vary from dermatologic to neuropathic, systemic or psychogenic causes. Treatment of persistent and intense itch is challenging and requires from the clinician the knowledge of the mechanisms behind it. Histamine is the main mediator in pruritus from cutaneous origin and thus, it responds well to antihistamines. However, once itch is caused by a systemic or neurogenic condition, our comprehension of its pathophysiology led us to a more targeted therapeutic approach, using opioid antagonists, antidepressants, ultraviolet B therapy, and androgens, among other drugs. Intractable itch deserves the same degree of attention as pain. It is a common problem and it causes as much discomfort as chronic pain. Based on recent studies, this work revises the neurophysiology behind each condition presenting with itch and aims at establishing a guidance on the diagnosis and treatment of pruritus.
