Abstract

[SPA]Los avances importantes en el ?rea de intervenciones terap?uticas y el tiempo transcurrido han justificado la actualizaci?n de la anterior ESPOGU?A. Las nuevas recomendaciones se centran en novedosos aspectos terap?uticos como son la incorporaci?n de las nuevas terapias biol?gicas para el tratamiento de la APs (los inhibidores de IL-23 e IL-17A/F o los inhibidores de JAK1,3 y JAK1) y las nuevas terapias para la EspAax-nr (inhibidores de IL-17A y IL-17A/F). Tambi?n se han incorporado nuevos datos sobre eficacia y seguridad de la inhibici?n de IL-17A en la APs axial; los resultados de los estudios head to head entre inhibidores de IL-17A e inhibidores del TNF-alfa; o los datos que sugieren que el metotrexato es eficaz en la entesitis y dactilitis de la artritis psori?sica. Asimismo, se ha recogido la evidencia actual proveniente de estudios retrospectivos que sugieren que un tratamiento de la psoriasis, sobre todo con terapia biol?gica, podr?a prevenir o retrasar la aparici?n de la APs; o tambi?n la actualizaci?n del debate sobre si la espondiloartritis axial y la artritis psori?sica axial son o no la misma enfermedad. Adem?s, se ha tenido en cuenta la carga que suponen las espondiloartritis en Espa?a y que el empoderamiento de los pacientes, para que participen en las decisiones y en la responsabilidad del manejo de su enfermedad, es muy importante, especialmente en aquellas medidas que mejoren la adherencia al tratamiento. De ah? la inclusi?n del papel de los profesionales de enfermer?a y la opini?n de los propios pacientes.
