Abstract

[ES]Introducci?n: El LE es una enfermedad inflamatoria cr?nica con afectaci?n predominante del ?rea anogenital. Afecta a ambos sexos pero las mujeres se ven afectadas con m?s frecuencia que los hombres. Su etiolog?a es desconocida aunque se cree que puede tener un origen autoinmune. Cursa con prurito, dolor, dispareunia, alteraci?n de la arquitectura anat?mica, empeoramiento de la calidad de vida y riesgo aumentado de degeneraci?n a CCE. Actualmente no existe un tratamiento curativo, el tratamiento est?ndar son los corticoides t?picos pero la recidiva es frecuente por lo que requiere terapia de mantenimiento que muchas pacientes no toleran. Objetivos: Realizar una revisi?n sistem?tica de las nuevas t?cnicas regenerativas aut?logas (PRP, injerto de grasa, terapia combinada) y dilucidar el papel que podr?an tener en el tratamiento del LEV. Material y m?todos: Para la revisi?n sistem?tica se han usado las bases de datos PubMed y Cochrane. Se han seguido las directrices PRISMA para su realizaci?n. Resultados: De los 147 art?culos iniciales se seleccionaron 8 para su an?lisis: 3 art?culos utilizaron PRP, 3 art?culos utilizaron injerto de grasa, 2 art?culos utilizaron la terapia combinada. Las tres t?cnicas mostraron mejor?a de la sintomatolog?a, calidad de vida y funci?n sexual de las pacientes tratadas as? como una disminuci?n de la necesidad de terapia de mantenimiento con corticoides. Conclusiones: Las terapias regenerativas basadas en la utilizaci?n de tejido aut?logo (PRP, injerto de grasa, terapia combinada) suponen una alternativa terap?utica prometedora en el tratamiento del LEV. Los estudios realizados hasta el momento han revelado datos esperanzadores, sin embargo, se hace necesaria la realizaci?n de estudios con tama?os muestrales mayores, seguimientos a largo plazo y presencia de grupo control con el que comparar los resultados.

[EN]Introduction:. LS is a chronic inflammatory disease with predominant involvement of the anogenital area. It affects both sexes but women are affected more frequently than men. Its etiology is unknown, although it is believed that it may have an autoimmune origin.It presents with pruritus, pain, dyspareunia, alteration of the anatomical architecture, worsening quality of life and increased risk of degeneration to SCC. Currently there is no curative treatment, the standard treatment is topical corticosteroids but recurrence is frequent so it requires maintenance therapy that many patients do not tolerate. Objectives: Conduct a systematic review of new autologous regenerative techniques (PRP, fat grafting, combination therapy) and elucidate the role they could play in the treatment of VLS. Material and methods: The PubMed and Cochrane databases were used for the systematic review. The PRISMA guidelines were followed. Results: Of the 147 initial articles, 8 were selected for analysis: 3 articles used PRP, 3 articles used fat grafting, 2 articles used combined therapy. All three techniques showed improvement in symptomatology, quality of life and sexual function of the treated patients as well as a decrease in the need for maintenance therapy with corticosteroids. Conclusions: Regenerative therapies based on the use of autologous tissue (PRP, fat grafting, combined therapy) represent a promising therapeutic alternative in the treatment of VLS. The studies carried out to date have revealed encouraging data. However, it is necessary to conduct studies with larger sample sizes, long-term follow-up and the presence of a control group with which to compare the results.
