Abstract

INTRODUCCI?N. La leche materna es la principal fuente de nutrici?n infantil durante al menos los seis primeros meses de vida. Algunos de sus componentes describen fluctuaciones circadianas que transfieren la informaci?n horaria de la madre al lactante, lo que convertir?a a la propia leche en un factor ?crononutritivo?. OBJETIVOS. El prop?sito de esta revisi?n es recopilar informaci?n sobre si hay componentes de la leche materna que var?an de manera circadiana y de cu?les se trata. METODOLOG?A. Las bases de datos utilizadas han sido PubMed y Scopus. Mediante una b?squeda sistem?tica durante los meses de febrero y marzo de 2024 y tras aplicar los criterios de elegibilidad se han revisado un total de 31 art?culos. RESULTADOS. M?ltiples componentes de la leche materna describen una ritmicidad circadiana, cada uno de ellos con un predominio diurno o nocturno y sus acrofases. Entre ellos se encuentran hormonas como el cortisol y cortisona (07:00h) , melatonina (03:00h), leptina (05:00h) adiponectina e insulina (10:00h), nucle?tidos como el 5?AMP (20:20h), 5?GMP (05:00h), 5?CMP (18:40h), 5?IMP (19:15h) y algunos amino?cidos entre los que destacan el tript?fano con un predominio nocturno (03:00h) y la metionina, con uno diurno. Estudios recientes confirman que hay otros componentes como las enzimas antioxidantes, citoquinas, vitaminas, minerales, c?lulas inmunol?gicas y endocannabinoides que tambi?n describen un patr?n circadiano. Los estudios muestran gran disparidad de resultados en cuanto a la ritmicidad de la concentraci?n de macronutrientes (l?pidos, prote?nas y carbohidratos). DISCUSI?N: Es evidente la necesidad de la estandarizaci?n de los procedimientos de muestreo. CONLUSI?N: Los componentes estudiados m?s importantes que presentan ritmicidad son los glucocorticoides y la melatonina, con un claro predominio diurno y nocturno, respectivamente. Los estudios realizados en los macronutrientes no arrojan resultados tan concluyentes.
