Abstract

La Diabetes Mellitus (DM) es un conjunto de patolog?as metab?licas cr?nicas, que se caracteriza por una hiperglucemia causada por defectos en la secreci?n de insulina, en su acci?n o en ambos. Presenta una mayor prevalencia con los a?os y conlleva complicaciones a corto y largo plazo. La Diabetes Mellitus Tipo 1 (DMT1) est? asociada a una destrucci?n de las c?lulas ? de origen autoinmune. Su tratamiento principal es la insulina subcut?nea ex?gena, sin embargo, esta no evita las complicaciones a largo plazo de la enfermedad. Por ello, se investigan nuevas l?neas de tratamiento, entre ellas, la terapia con c?lulas madre, c?lulas indiferenciadas, con capacidad de autorrenovaci?n y de diferenciaci?n. Nuestro objetivo es estudiar el tratamiento de la DMT1 con c?lulas madre. Para ello hemos llevado a cabo una b?squeda en PubMed con los t?rminos m?dicos ?T1DM?, ?Diabetes mellitus, type 1? y ?Cell -and tissue- based therapy?, a?adiendo los filtros ?Clinical Trial? y ?Systematic Reviews?. Los criterios de inclusi?n fueron: estudios cl?nicos aleatorizados y no aleatorizados, revisiones bibliogr?ficas sistem?ticas, sobre el trasplante de c?lulas madre, con datos estad?sticos completos, pacientes con DMT1, art?culos en ingl?s o espa?ol y fecha de publicaci?n en los ?ltimos 10 a?os. Definimos la eficacia como la independencia a la insulina tras el trasplante y la toxicidad en t?rminos de efectos adversos. Tras nuestra revisi?n concluimos que el trasplante con c?lulas madre hematopoy?ticas es m?s eficaz que el de las c?lulas madre mesenquimales y del cord?n umbilical; y la terapia con c?lulas madre en DMT1 es segura.
