Manejo inicial del paciente con herida por arma de fuego

Abstract

Las heridas por arma de fuego pueden provocar da?os muy distintos dependiendo del intercambio de energ?a entre el cuerpo y la bala, adem?s todas ellas tienen un orificio de entrada, una trayectoria y puede tener o no un orificio de salida que depender? de que el proyectil se quede dentro del cuerpo o no. El manejo inicial de estas heridas supone poner en marcha ciertos mecanismos con el fin de asegurar la v?a aerea (VA), la respiraci?n, control de la hemorragia y reposici?n de volumen, hoy en d?a prima actuar primero ante la circulaci?n antes que ante la v?a a?rea y respiraci?n. Con el fin de controlar la hemorragia, hay que seguir una secuencia: primero hay que intentar controlarla con presi?n directa junto a vendajes compresivos, si no es eficaz se realiza torniquete o aplicaci?n de agentes hemost?ticos, dependiendo de donde se localice el punto de sangrado. Hay mucha controversia ante la utilizaci?n del torniquete, pero se ha demostrado que es una aplicaci?n correcta no conlleva riesgos. La fluidoterapia y la hipotermia son dos aspectos que tambi?n hay que considerar en este manejo inicial, ya que si no los atendemos, el pron?stico y la gravedad del paciente puede empeorar cuantiosamente.
