Seguimiento farmacoterapéutico a pacientes diabéticos

Abstract

[ES]En este trabajo se presentan dos casos cl?nicos de seguimiento farmacoterap?utico a pacientes diab?ticos, utilizando la metodolog?a D?der. Se valora el estado cl?nico de los pacientes a trav?s de una entrevista farmac?utica y se estudia su situaci?n. Posteriormente se valora la Necesidad, Efectividad y Seguridad de cada uno de los tratamientos del paciente, para as? detectar posibles problemas relacionados con los medicamentos, resultados negativos asociados a la medicaci?n o puntos de mejora de su tratamiento farmacol?gico y no farmacol?gico. Una vez decididas las posibles intervenciones a realizar, en las siguientes entrevistas con el paciente se acuerdan las medidas a llevar a cabo. En los dos casos presentados, la educaci?n sanitaria fue esencial para asegurar la adherencia y mejorar los resultados de los tratamientos farmacol?gicos. Adem?s, se estableci? un plan de seguimiento para optimizar la terapia de los pacientes de forma peri?dica y ante cualquier cambio sustancial de su tratamiento o situaci?n cl?nica.

[EN]In this paper, two cases of pharmaceutical care intervention to diabetic patients are presented, using the Dader methodology. At first, the clinical status of the patients was assessed through a pharmaceutical interview. Later, Necessity, Effectiveness and Safety of treatments of each patient was evaluated in order to detect possible drug-related problems, negative outcomes associated to medication or areas for improvement of pharmacological and non-pharmacological treatment. In the following interviews with the patients, we decided with them which interventions we'll make. In the two cases presented, health education was essential to ensure adherence and improve outcomes of pharmacological treatments. In addition, we started a monitoring plan that will serve to optimize the therapy of patients regularly and at any change of the treatment or clinical status.
