La cricotirotom?a es el procedimiento m?s r?pido de acceso a la v?a a?rea. Objetivo: Exponer un algoritmo de decisi?n. Material y m?todo: Revisi?n bibliogr?fica. Comentarios: Se describe la t?cnica de cricotirotom?a quir?rgica (por incisi?n) y el algoritmo de decisi?n que empleamos en pacientes con obstrucci?n de la v?a a?rea con ventilaci?n ? intubaci?n imposible. Conclusiones: Recomendamos la cricotirotom?a quir?rgica (por incisi?n) como la primera opci?n porque necesita de pocos elementos disponibles en cualquier unidad hospitalaria (bistur? del n? 21?22 y tubo endotraqueal de 4,5?5mm) y se realiza en pocos segundos en 4 pasos. [EN] Introduction: In patients with difficult airway with ventilation failure and intubation impossible, cricothyrotomy should be performed. Cricothyrotomy is the fastest procedure access to the airway. Objective: To present a decision algorithm. Material and methods: Literature review. Comments: We describe the surgical cricothyrotomy technique (incisional) and our decision algorithm we use in patients with obstruction of the airway with ventilation - intubation impossible. Conclusions: We recommend surgical cricothyrotomy (incisional) as the first option because it needs a few elements available in any hospital unit (scalpel No. 21-22 and endotracheal tube 4.5-5mm) and only takes a few seconds in 4 steps.
